The Post-9/11 Military2VA (M2VA) Program Manager oversees all transition and coordination of care activities for Post 9/11 Servicemembers and Veterans (SM/Vs) treated at the facility. Their health care and psychosocial problems are complex and require a high degree of clinical oversight and creative problem solving.
This position has programmatic responsibility and may have administrative responsibility for facility staff and transition patient advocates who provide services to Post 9/11 SM.

Serves as the Point of Contact (POC) and Coordinator for the Post-9/11 M2VA Program and assumes the technical and administrative responsibility for management of the program. Including, but not limited to:
Program Management - Provides administrative oversight of the POST-9/11 M2VA Program to include planning, establishing, and implementing policies and procedures; developing goals and objectives; and monitoring, operating, and evaluating the functioning of the program.
Follows Veterans Health Administration (VHA) policy and develops systems to identify and track post 9/11Servicemembers/Veterans (SM/Vs) served by the facility and community.
Ensures that severely-injured and/or ill post 9/11 SM/Vs and other SM/Vs with case management needs are assigned to a case manager and works closely with care management teams.
Serves as the POC for inquiries regarding post 9/11 SM/Vs and for post 9/11 SM/Vs seeking health care.
Coordinates programs and services for health evaluations for program participants.
Promotes and coordinates evaluations for the POST preventive 9/11 M2VA mental and physical Program in the facility and Community Based Outpatient Clinics (CBOCs).
Prepares reports for the VA Central Office, VISN POC, and Medical Center leadership.
Monitors referrals between the VA medical center, Military Treatment Facility and/or community hospitals to promote appropriate and timely transfers.
Establishes standards for the POST 9/11 M2VA Program that promote quality and efficiency in coordination with the goals of the medical center, the VISN, and VA Central Office
Ensures compliance with Joint Commission (JC), Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards.
Researches community resources available to provide continuity of care to enhance the quality of life of the Veteran. Disseminates information on resources throughout the VISN.
Explores community resources including family friendly delivery systems to Identify unmet needs of the post 9/11 population and works with facility, VISN, and VA Central Office to develop and implement solutions.
Develops a data driven continuous improvement program to evaluate and document the program's effectiveness.
Develops and implements plans for outreach to post 9/11 SM/Vs who do not presently use VA for their health care.
Ensures that the POST-9/11 M2VA Program is interdisciplinary and integrated with the medical programs.
Establishes relationships with providers to foster involvement in the care of Veterans in the POST-9/11 M2VA Program.
Manages the daily operation of the POST-9/11 M2VA Program and provides accountability for effectiveness of service.
Identifies gaps in services and develops a plan with staff and other departments to provide the best resources to meet program needs.
Oversight Of Case Management Services - The POST-9/11 M2VA Program Manager is responsible for and may supervise POST-9/11 M2VA nurse and social worker case managers and facility Transition Patient Advocates.
Ensures that all Post 9/11 SM/Vs are screened, and those in need of nurse and/or social worker case managers are assigned accordingly.
Coordinates the assembly of the Inter-Disciplinary Care Management Review Team (IDCMRT).
Manages, coordinates, and advocates for individual and family treatment services as needed.
Ensures veterans are referred to other organizations and resources for services not available from the facility.
Maintains a network of referral resources to include substance abuse treatment, outpatient medical and/or psychiatric care, vocational rehabilitation, etc. and with VSOs.
Monitors and reviews CMTRA updates for compliance with the determined contact plan.
Oversees the Transition Patient Advocates, including monitoring their caseload and the advocacy services provided to post 9/11 SM/V.
Advocates for the post 9/11 SM/Vs to ensure services and benefits are obtained in a timely manner and to meet the VA's goal for excellence in customer service.
Works collaboratively with Vet Centers and the Veterans Benefits Administration (VBA).
Patient, Stakeholders, and Staff Education - Provides consultation to other staff on the special needs of Post 9/11 SM/Vs.
Provides education to Post 9/11 SM/Vs on VHA health care services at the facility and within the VISN and local community.
Establishes and maintains education programs to facilitate understanding of medical treatment, including clinical practice guidelines, mental health issues, readjustment issues, psychosocial problems, and care giver issues facing the Post 9/11 SM/V population.
Work Schedule: Full-Time, Monday - Friday, 0730-1600 or 0800-1630

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: Social Worker, GS-12
Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Experience serving service members in the military. Case Management experience.

Physical Requirements: The work is primarily sedentary but requires distance driving. It may require some occasional lifting (10-35Ibs). Requires reaching above shoulder, twisting neck with ability to see from side to side with range of motion at least 45 degrees, and ability to coordinate all four extremities. Position requires frequent keyboarding (4-7 hours per day), sitting (4-7 hours per day), walking up to 6 hours per day, or standing up to 8 hours a day. Infrequent climbing/ descending of stairs and some bending. Ability for rapid mental and muscular coordination simultaneously. Requires incumbent pass a pre-employment driving physical through Occupational/Employee Health to transport Veterans in a government vehicle.
