VA Portland Health Care System (VAPORHCS) is a Level 1A major interdisciplinary teaching and tertiary facility with service-lines serving primary care, emergency medicine, medicine, surgery, specialty, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and extended care inpatients and outpatients throughout Oregon and Southwestern Washington. Positions is located in Vancouver, Washington.
Duties
The Home Based Primary Care (HBPC) Team Manager, Supervisory Social Worker, is located within assigned to Home & Community Based Services Department (HCBS) in the Rehabilitation & Long Term Care Division (RLTC) Division and supervises the Vancouver HBPC Interdisciplinary Team. Functions include:
Supervise and evaluate day-to-day activities and performance of the members of the interdisciplinary team
Develop, assess, and modify tasks, functions, procedures, and systems to accomplish the objectives of the HBPC program and the HCBS Department experience, and patient care outcomes
Engage in VA and community staff education regarding HBPC, GEC, and Non-Institutional services and referral processes
Development, evaluation, and communicate standards, policies, and procedures
Evaluate work performance, counsel employees whose performance is marginal or unsatisfactory, initiate disciplinary action when necessary, identify employees deserving of awards or advancement
Additional duties as required
Work Schedule: Monday - Friday; 7:30 am - 4:00 pm or 8:00 am - 4:30 pm

Qualifications

BASIC REQUIREMENTS:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. http://vaww.va.gov/OHRM/T38Hybrid/
PREFERRED EXPERIENCE:
Must have a valid driver's license and proof of insurability
Home health experience preferred
5 or more years med/surg experience preferred
Managerial/supervisory experience preferred

Supervisory Social Worker, GS-12:
Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below.
Demonstrated Knowledge, Skills, and Abilities:
Ability to independently organize work, set priorities, and meet multiple deadlines.
Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
Ability to ensure provision of clinical social work services.
Ability to provide staff training and development.
Skill in interpersonal relationships in dealing with employees, team leaders, and managers.
PREFERRED EXPERIENCE:

Home health or primary care experience preferred
15 or more years of medical social work/surg experience preferred2. Home health experience preferred

The full performance level of this vacancy is GS-12.
PHYSICAL REQUIREMENTS: Moderate lifting, 15 -44 pounds, light carrying, under 15 pound, use of fingers, both hands, walking (1 hour), standing (1 hour), both legs required, operate motor vehicle, ability for rapid mental and muscular coordinating simultaneously, visual at least 20/40 each eye, both eyes required, ability to distinguish shades of color, hearing aid permitted, at least 70 peripheral in horizontal, mental and emotional stability.
