The Clinical Pharmacy Specialist (Cardiology Service and Patient Aligned Care Teams (PACT)) position is located in the Main Hospital in Cardiology Service and the PACT Clinic. The primary responsibility will encompass the provision of evidence-based pharmaceutical care services through direct care with patients and consultation with other providers and health care staff in these settings. Services are conducted via multiple modalities, including virtual and in-person care.

Core activities outlined within the scope of practice include: Executing therapeutic plans utilizing the safest and most cost-effective medication treatments Performing the physical measurements and objective assessments necessary to ensure the patient's appropriate clinical responses to therapy Ordering, subsequent review, and action on appropriate laboratory tests and other diagnostic studies necessary to monitor, support, and modify the patient's drug therapy Ordering and prescribing medications, vaccines, devices, and supplies to include: initiation, continuation, discontinuation, monitoring, and altering therapy Ordering consults and referrals, as appropriate, to maximize positive drug therapy outcomes; examples include, but are not limited to, referrals for routine care or assessment to clinical nutrition professionals, prosthetics, social workers, integrated mental health professionals, or other specialty areas Obtaining and documenting informed consent for treatments and procedures that require consent for which the clinical pharmacist is responsible, including those where the clinical pharmacist is the prescriber of a treatment that requires consent or when they are providing medication management services on behalf of the original prescriber NOTE: The clinical pharmacist must have sufficient knowledge and training in the treatment or procedure, its indications, risk and benefits, complications, and alternative treatments, to effectively counsel the patient Conducts virtual and in-person (when appropriate) encounters with patients for CMM in the PACT practice area Achieves minimum productivity expectations as set forth by Associate Chief of Pharmacy for Clinical Services based on Clinical Pharmacy Practice Office (CPPO) guidance Follows-up with patients in a timely manner and monitors for drug therapy response, potential adverse drug reactions, allergies, compliance, laboratory results, and other monitoring measures as appropriate; treatment plan adjustments are made accordingly based on assessed measures Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, pharmacokinetic impact, and therapeutic outcomes as required; information is communicated with team members and appropriate alternatives to current treatment plans are provided Promotes and monitors compliance for established drug therapy policies PROGRAM MANAGEMENT/ADMINISTRATIVE: Collaborates in the development and maintenance of policies and procedures defining operational requirements for the clinical pharmacy program Completes administrative projects and reports in a timely manner Serves on VISN and/or facility level committees as requested Adheres to and ensures the pharmacy service is compliant with all regulatory and accrediting body requirements, as well as PACT related policies and procedures EDUCATION AND RESEARCH: Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents and other staff as assigned Serves as a preceptor for pharmacy students and pharmacy residents in accordance with established policies Attends and actively participates in pharmacy related training, in-services, staff development, and relevant conferences Serves as an advocate for the pharmacy profession and educates team members on PACT and CPS provider processes Assists in the training of new PACT CPS, competency assessments, and mentoring as requested Responsible for keeping abreast of changes in the Pharmacy Benefits Management (PBM), CPPO, and PBM Formulary services (e.g., formulary additions and deletions, relevant memoranda, and policies and procedures), as well as general knowledge of VA, Drug Enforcement Administration, US Food and Drug Administration, and The Joint Commission regulations Prepares and presents educational lectures to a variety of disciplines as required Participates in PACT related evaluation studies as requested Other duties as assigned Work Schedule: Monday through Friday, 8am - 4:30pm Telework: Not Authorized Virtual: This is not a virtual position. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Grade Determinations: GS-13 Experience or Education: In addition to the GS-12 requirements, MUST have one (1) full year of experience equivalent to the next lower grade level. Examples of GS-12 level equivalent experience are but not limited to: Knowledge of professional pharmacy practice; Ability to communicate orally and in writing to both patients and health care staff; Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security; and Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Clinical Pharmacy Specialist: The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. AND, In addition to meeting the GS-12 KSA, Pharmacists assigned to this position must demonstrate the following KSAs at the GS-13: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e.
