This position is located at the Yankton Service Unit (YSU), Wagner, SD. The purpose of this position is to provide patient case management by a nurse specialist, registered nurse, for an identified patient population, to ensure continuity, coordination, and comprehensive management of patient care, and to promote cost effectiveness of care through utilization and application of available resources. Promotes the Patient Centered Medical Home (PCMH) concept in all aspects of nursing care.
Duties
Directly supports patients, providers, and clinic staff.
Provides case management for patients by coordinating services to meet patient care goals and conducts utilization review meetings as needed.
Serves as primary point of contact for outside facilities who provide care for IHS patients. Active participant in discharge planning and facilitates continuity of care.
Pro-actively supports and helps facilitate the Patient Centered Medical Home model and SD State Health Home Initiative for the service unit.

MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level (GS-9) in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care.
