This position is located at the Yankton Service Unit (YSU), Wagner, SD. The purpose of this position is to provide patient case management by a nurse specialist, registered nurse, for an identified patient population, to ensure continuity, coordination, and comprehensive management of patient care, and to promote cost effectiveness of care through utilization and application of available resources. Promotes the Patient Centered Medical Home (PCMH) concept in all aspects of nursing care.

Duties

Directly supports patients, providers, and clinic staff. Provides case management for patients by coordinating services to meet patient care goals and conducts utilization review meetings as needed. Serves as primary point of contact for outside facilities who provide care for IHS patients. Active participant in discharge planning and facilitates continuity of care. Pro-actively supports and helps facilitate the Patient Centered Medical Home model and SD State Health Home Initiative for the service unit.
