Located within Mental Health Service at Eastern Oklahoma VA Healthcare System (EOVAHCS), incumbent serves as Primary Care Management Module Coordinator (PCMM) for Mental Health Services. Incumbent helps with training, implementation, monitoring and evaluation of all administrative procedures related to MH PCMM operations.PCMM is a complex, nationally implemented, standalone computer program designed to monitor and manage mental health services throughout the VA system.

Duties

Responsible for maintaining integrity of the PCMM database for all Mental Health Services throughout the hospital service area Incumbent will serve as a contact point for all new referrals to Mental Health Services; entering Mental Health information into PCMM system; enrolls patients into Mental Health teams; assigns patients to appropriate provider within the PCMM system; removes patients from database when discharged; maintains accurate enrollment data, identifies and corrects discrepancies in database system; and discharges from multiple or inappropriate specialty and consult clinics Incumbent has primary responsibility for establishing and managing PCMM team and position set up for all necessary Mental Health Service teams supervised by this Medical Center. Specifically; electronically creates or modifies team and position settings; assigns provider user class based on verified credentials; links associated clinic, sets maximum panel sizes; programs clinical alert notification levels; and establishes preceptor to associate provider links Incumbent will be responsible for integrity of the Medical Center's significantly large PCMM database for Mental Health Services Incumbent performs work involving the collection, compilation, and tracking of data in support of the organization's program operation (i.e. using computerized patient lists to call and verify or update current patient information) Receives and processes documents/reports in accordance with established guidelines and procedures; reviews documents and reports for completeness and enters pertinent information into an automated tracking system Incumbent will be required to work closely with Medical Center clinic support staff, clinical Service Chiefs, CBOC staff and contract providers, DSS staff, VISN staff, and VACO staff; provides reports pertaining to this data to committees and work groups assigned to analyze and evaluate the effectiveness and efficiency of the PCMM database; includes monitoring and maintaining provider profiles, monitoring panel sizes, oversight and correction of transmission errors; review/rectification of incorrect patient assignments; assign/reassign patients based upon clinician documentation, AD HOC reports, Fileman routines; manages inactivation list which includes deaths, discharged patients, transfers Incumbent's work involves the collection, compilation, and tracking of data in support of organization's program operation (i.e. using computerized patient lists to call and verify or update current patient information); receives and processes documents/reports in accordance with established guidelines and procedures; reviews documents and reports for completeness and enters pertinent information into an automated tracking system; presents data from the PCMM program as needed by Medical Center and VISN management Incumbent tracks/follows up on any action items assigned by mental health leadership related to PCMM; works closely with Facility and VISN Clinical Applications Coordinators (CACs) as well as IRM staff to produce monthly reports regarding provider specific quality measures displaying historical, cumulative, and comparative data Work Schedule: Monday - Friday; 8:00am - 4:30pm Virtual: This is eligible for Virtual assignment Position Description/PD#: Program Support Assistant - Primary Care Management Module Coordinator/PD140450 Financial Disclosure Report: Not required

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/10/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. Preferred Experience: Veterans' Administration, VISTA, and CPRS experience is preferred but not mandatory. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-06 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: timekeeping, purchase cardholder, knowledge preparing committee agenda/minute preparation, Primary Care Management Module (PCMM) Software Management and Compensation and Pension (C&P) Exam Scheduling, VISTA, and CPRS. Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements and Work Environment: Work is sometimes sedentary but requires walking, standing and carrying of medium weight items, such as, patient records. Incumbent works extensively in the hospital computer system. Incumbent must have an even temperament and act in a professional manner during work performance. The employee works with team members as well as various other facility personnel. The employee may encounter patients, with which they are assigning and should be informed of how to handle such patients if the need arises (i.e. Suicide Prevention Techniques, Psychiatric Emergency Room contacts, etc.) The work area is adequately lighted, heated, and ventilated.
