The Caregiver Support Coordinator (CSC) is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence based services and interventions; program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality improvement activities; and evaluation/consultation.

Duties

The Caregiver Support Coordinator develops and implements the Caregiver Support Program specific to his/her area of assignment and insures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives. Incumbent will execute the rollout of the Caregiver expansion related to the VA Mission Act of 2018. The incumbent administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families and Veterans. Develops and implements information systems to track service activities including visits, referrals, caregiver demographics, equipment utilization and caregiver and Veteran satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. The incumbent participates in direct caregiver and Veteran contacts to identify and assess caregiver stress/burden and the development and application of directed clinical treatment interventions. Advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues. Creates educational tools, develops programs and implements training focused on specific caregiver needs/issues. Provides education and in-service training to VAMC employees, Veterans, families and caregivers. Provides ongoing consultation and support to colleagues regarding caregiver support. Develops and facilitates caregiver support groups (face-to-face or VANTS telephone line) and educational programs. Distributes educational materials provided by VA, local, state and national caregiver organizations. Coordinator administers information to enhance the quality of services provided to veterans. The incumbent develops and implements information systems to track CareGiver Service activities including visits, referrals, patient demographics, patient satisfaction, and resources as required and needed. In collaboration with the Caregiver Program Manager/Supervisor, the incumbent develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Responsible for the oversight of completion of required authorization documentation, monthly data collection, as well as forms for evaluation and monitoring of service delivery and outcome. Serves as a liaison to community agencies for networking and resource development, maintaining effective contacts with numerous social, health, and civic agencies. Provides support and acts as a consultant to facility wide staff to provide education and support for securing CareGiver Services. Develops a data driven continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness. Establishes collaborative relationships with physicians, nurses, and other disciplines to foster their knowledge and referrals to veterans requiring CareGiver Services. Manages the daily operation of the CareGiver Support Program and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfaction. Conducts caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues. Collaborates with the Patient Aligned Care Team, mental health team and staff coordinating care with assigned Dyad to promote coordination of care and facilitate recovery. Develops a library of caregiver support information material for distribution through the various inpatient/outpatient and specialty clinic settings. Educational material must be specific to the age group serviced in the clinic, i.e. Alzheimer's disease and Parkinson disease in the general and geriatric settings and PTSD and TBI information in the general, OEF/OIF, post-deployment and Polytrauma Center settings Develop a data monitoring system to measure the efficacy of the program. Coordinates the events and programs for the annual National Family Caregivers Month every November that includes announcement and advertising of month long activities, mobilization of participants, educational sessions, caregiver recognition events and caregiver support programs. Identify Caregiver Stress and make appropriate referrals for VA and community resources. Referrals may include respite care, adult day care and community nursing home programs. Manages caseload of approved Family Caregivers (Dyads) including monitoring visits, initial applications, oversight of current approved applications, caregiver stress and maintaining active communication with the Dyad. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Grade Determinations: At the GS-12 grade, you must possess one year of specialized experience equivalent to the GS-11 level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs below: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is the GS-12 Physical Requirements: The social work duties include sedentary and ambulation (walking) throughout the medical center and in the community according to the demands of daily tasks. Light carrying, less than 15 pounds-may be required as part of duties related to storing or moving files. Working around moving objects or vehicles-required as part of daily ambulating within the medical center around patients who use motorized scooters and wheelchairs. Working along-duties require working along at intervals during the day (i.e., documenting progress notes, completing phone call, etc.). Reaching above shoulder- may be required during daily task to access files located in overhead cabinets. Walking- required approximately 3-4 hours daily associated with walking from assigned office to outpatient clinics and other medical center departments as needed to complete duties of the position. Standing-required approximately 3-4 hours daily associated with walking from assigned office to outpatient clinics and other departments within the medical center as needed to complete duties of the position and filing patient records as needed. It is required that this individual be able to operate a motor vehicle as this position and it is expected that the incumbent visit patients off-site as part of their duties. Work will take place in- and outdoors thus exposing them to the natural elements of the weather.
