Duties

***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** The major duties include but are not limited to: Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones; Conducts interventions for Veterans using one-on-one, couple or group modalities in outpatient and inpatient settings; Provides eligible Veterans and family members with information regarding lifestyle management including weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, pain management, mental health, etc.; Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments; Reviews current health behaviors and develops customized treatment plans with Veterans; Schedules ongoing sessions to assess progress and assist in navigating obstacles or challenges; Coordinates care with Veterans' treatment providers, both primary and specialty care; Works collaboratively within a team setting to provide positive outcomes for participants, peers and overall operations; Conducts outreach activities to inform Veterans and family members of available programs; Develops and distributes locally approved health education materials to identified enrolled Veterans and family members; Participates in leadership roles on local, state and/or national professional organizations specific to health coaching; Assists with data management analyses, special studies and reports for use by management for budget preparation, workload projections and quality assurance and utilization reviews; Participates in the establishment of administrative standards locally; the objective of which is intended to improve programs, facilities and operations; and, Performs all other duties as assigned. Work Schedule: Monday-Friday; 8:00am-4:30pm Recruitment & Relocation Incentives: Not authorized Financial Disclosure Report: Not Required DESIGNATED DRUG-TESTING POSITION: Applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. English Language Proficiency: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position. ~AND~ Basic Requirements: You must meet one of the basic requirements below to be eligible for the Social Science Program Coordinator - Whole Health Coach, GS-0101. Degree: You must have successfully completed a bachelor's degree or higher degree, with a major study in behavioral or social science or related disciplines appropriate to the Social Science Program Coordinator - Whole Health Coach position. Education cannot be credited without documentation. ~OR~ Combination of education and experience: You must have a combination of education and experience that provided you with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. You should possess a core of educational credit in behavioral science, social science or a related discipline plus additional education and/or experience. The quality of the combination of education and experience must be sufficient to demonstrate that you possess the knowledge, skills and abilities required to perform work in the Social Science Program Coordinator - Whole Health Coach position and is comparable to that normally acquired through the successful completion of a full 4-year course of study with a major in the appropriate field. Experience must be fully documented on your resume and must include job title, duties, month and year start/end dates AND hours worked per week. ~OR~ Experience: You must have at least four (4) years of appropriate experience that demonstrated that you have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. NOTE: Experience must be fully documented on your resume and must include job title, duties, month and year start/end dates AND hours worked per week. ~AND~ In addition to the English Language Proficiency and meeting one of the Basic Requirements above, to qualify for this position at the GS-9 level, you must meet one of the following: Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-7) in the Federal Service that has given you the particular knowledge, skills and abilities required to successfully perform the duties of a Social Science Program Coordinator - Whole Health Coach, and that is typically in or related to the work of the position to be filled. Specialized experience includes: Assisting clients with developing and achieving self-determined goals related to health and wellness; Utilizing motivational interviewing and health coaching techniques and processes to assist clients in developing a realistic plans to accomplish their goals; Developing treatment plans for clients focused on behavioral interventions; Conducting ongoing sessions to assess progress and assist clients in navigating obstacles or challenges; Conducting interventions for clients using one-on-one, couple, or group modalities; and Assisting in the collection of whole health qualitative data from clients and creating reports analyzing effectiveness of health coaching. NOTE: Experience must be fully documented on your resume and must include job title, duties, month and year start/end dates AND hours worked per week. ~OR~ Education: Successfully completed two (2) years of progressively higher level graduate education leading to a master's degree or masters or equivalent graduate degree that provided the knowledge, skills and abilities necessary to do the work of the Social Science Program Coordinator - Whole Health Coach position. Such education may include courses in behavioral or social science, or related disciplines appropriate to the position. NOTE: Transcripts (unofficial or official) must be submitted with your application materials. Education cannot be credited without documentation. ~OR~ Combination: A combination of successfully completed graduate level education (beyond the first year) and specialized experience, as described above, which meet the total experience requirements for this grade level. This education must provide the knowledge, skills and abilities necessary to do the work of the Social Science Program Coordinator - Whole Health Coach position. Such education may include courses in behavioral or social science, or related disciplines appropriate to the position. You will be rated on the following Competencies as part of the assessment questionnaire for this position: CommunicationData ManagementLeadershipTechnical Competence IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Part-time experience will be credited on the basis of time actually spent in appropriate activities.
