The incumbent serves as a core member of the Behavioral Health Interdisciplinary Program (BHIP) Team and is responsible for coordinating continuum of care transitions to and from the outpatient mental health clinic and other programs/service areas. Works independently to provide expert level psychosocial interventions and case management services to veterans with complex medical, social and behavioral conditions to enhance the delivery of care and improve mental health treatment outcomes.

The (OMH) Clinical Social Work Case Management Coordinator provides a full range of Social Work services within commonly accepted standards of Social Work practice, which includes case work, individual, family and group counseling, care coordination, case management, and other services as appropriate.
Duties include but are not limited to:
Provides expert level of therapeutic intervention to identify and address goals of rehabilitation, recovery, health maintenance, improved quality of life, and community integration.
Utilizes advanced practice skills, knowledge, and judgment in developing and measuring effectiveness of social work practice and services, utilizing outcome evaluations to improve treatment services.
Has specialized knowledge of clinical care coordination, case management and psychosocial treatment of patients with severe mental illness, PTSD, addictive disorders/dual diagnosis, and other significant mental and physical health conditions.
Assists in the development and implementation of service agreements/memorandums between OMH and other program and service areas.
Analyzes and synthesizes data associated with transitions of care and utilization of mental health services.
Develops and coordinates specialized psychosocial services and programs within OMH.
Independently provides consultation services to other staff and programs regarding the psychosocial needs of veterans and the impact of psychosocial problems on the treatment and management of mental health conditions.
Coordinates and schedules referrals for case management services.
Meets weekly via conference with JHQVAMC MH Care Coordinators to coordinate ongoing implementation policies and procedures related to referrals.
Attends all appropriate staff meetings.
Completes all administrative and/or treatment activities assigned by the ACOS, Mental Health, Deputy ACOS for Mental Health, and the MH Outpatient Services Coordinator.
Provides screening, case management and crisis intervention assistance to Veterans seen for acute and/or urgent mental health care needs and special patient populations.
Works within the interdisciplinary treatment team to develop a treatment plan with the Veteran based on the assessment.
Utilizes assessment tools and assessment methods that best serve the Veteran.
Expert knowledge of veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension and facilitates and prioritizes referrals based upon veterans needs and eligibility.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency Requirement - Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English.
Education - Candidates must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure - Social workers at the GS-12 must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Experience: In additional to meeting the basic requirements as stated above, you must have the following:
GS-12 Level: Social Worker (Case Management Coordinator) must posses at least One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience, and are able to demonstrate the following knowledge, skills and abilities:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience:
Experience working with crisis clinics and the Veteran population.

The full performance level of this vacancy is GS-12.
