This ARNP provides direct patient care in the following areas, as determined by the needs of the Behavioral health Service: Comprehensive Case Management Team, Substance Use Disorder Treatment Program, Homeless Program, and Outpatient Psychiatric Care. Provides comprehensive outpatient medical evaluation and follow-up care to Veteran patients within these programs at the Mann-Grandstaff VA Medical Center in Spokane, Washington.

Duties

Duties are, but not limited to: Clinical Responsibilities: Executes position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex patient care. Time in this role also includes actively fostering collaboration and consultation with mental health and medical providers of all disciplines who support psychiatric care. Participates actively in training of psychiatry residents, psychology interns, primary care staff, and others. Responsible to document care in the agency's electronic medical record and participates in quality assurance activities and peer chart reviews. The ARNP is an expert nurse, "with an enormous background of experience, who now has an intuitive grasp of each situation and zeros in on the accurate region of the problem without wasteful consideration of a large range of unfruitful, alternative diagnoses and solutions (Benner)." Demonstrates performance and leadership that is broad enough to improve the care for a diverse group of patients. The ARNP is a generalist Advanced Practice Nurse with broad experience and familiarity of a wide range of medical conditions, who provides a focused range of medical care appropriate to an outpatient setting, including urgent and emergent evaluation of episodic and chronic problems; comprehensive, integrated, Veteran-centered general medical care; and collaborative intervention in concert with a multidisciplinary hospital team and with patients' Primary Care Providers. The scope is further defined in approved clinical privileges for Advance Practice Nurses Supervisory Responsibilities: 1. Provides clinical oversight of general outpatient psychiatric care at the Spokane VAMC-highly professional services that are critical components of the Medical Center's health care mission. Incumbent is responsible for the quality of outpatient psychiatric evaluation, intervention, and consultation offered by physicians, mid-level prescribers, and others. 2. Participates in the development of policies and procedures for psychiatric care and consultation offered at VAMC Spokane. 3. Coordinates and provides clinical oversight of physicians, nurse practitioners, and other professionals who provide or support provision of psychiatric care in the general outpatient areas. 4. Completes formal annual performance appraisals of all supervisees. 5. Promotes clinical training and development of psychiatry staff and those who support provision of psychiatric care. 6. Assures that all supervisees are competent to practice as privileged. Assists medical staff office with privileging process for psychiatrists new to VAMC Spokane. Completes OPPE's and FPPE's for outpatient physicians and ARNPs. Monitors and reports to the Psychiatry Section Chief on the clinical competency of all those providing psychiatric care. 7. In collaboration with the Psychiatry Section Chief, develops and proposes systems for assessing and monitoring clinical outcomes for psychiatric care, as well as for improving clinical practices through outcome data. 8. Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License and Certification: License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared. NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (NP) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) II: Exception to Licensure for Graduate Nurse Technicians. Candidates who otherwise meet the basic requirements, but do not possess the required licensure and/or certification, may be appointed at the entry step of the Nurse II, as a Graduate Nurse Technician - Nurse Practitioner on a temporary appointment up to any period not-to-exceed one year and a day under the authority of 38 U.S.C. § 405(c)(2)(B). Scope: Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others Education and Experience Requirement: Meets basic education requirement for APN (NP). Nurse II is considered the entry grade level for APN (NPs); no experience is required. Dimension Requirement: APNs (NPs) at level II are expected to meet the following dimension requirements: Clinical Practice. Delivers evidence-based health care, demonstrating proficiency in the ability to manage a patient panel, population, or program. Outcome Focus. Utilizes research and evidence-based practice to evaluate the patient care delivery system within a patient panel or program, presents findings and contributes to changes that enhance the quality and timeliness of veteran/patient care. Leadership. Works effectively as a leader within a team/unit/group and seeks interdisciplinary collaboration as appropriate to enhance patient care. Professional Development. Acquires knowledge and skills to develop expertise in area of practice. Professionalism/Collegiality. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Participates in interdisciplinary teams and processes that positively impact patient outcomes. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) III: Scope: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Education and Experience Requirement: Meets basic education requirement for an APN (NP) AND one year of specialized APN (NP) experience equivalent to the Nurse II grade level. Dimension Requirement: APN (Nurse Practitioners) at grade III are expected to meet the following dimension requirements: Clinical Practice. Influences and models the delivery of evidence- based health care, demonstrating expertise in the ability to manage a patient panel, population, or program. Outcome Focus. Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations. Leadership. Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program. Professional Development. Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility. Professionalism/Collegiality. Initiates and leads interdisciplinary teams developing systems that positively impact outcomes at the panel, population, or program level. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Preferred Experience: Advanced education and clinical training, provide comprehensive primary and/or specialty health care services for groups of patients. Nurse Practitioners, using the nursing process. Direct and coordinate the provision of health care services for patients in primary care, special projects, specialty clinics, across service lines, and within the medical center or VISN. Provide a model for clinical practice within the service line, medical center or within the community. Functions as a role model in providing age-specific care by recognizing a person's commitments and functions in life, supporting the patients right to make informed choices, and assisting the patient in coping and problem-solving with respect to their current health status relative to their age. Demonstrates the ability to assess data reflective of the patient's status and care requirements for population-specific needs, such as, culture, veteran era, etc., and plans and delivers care appropriately Serves as a resource and advocate in resolving legal/ethical issues such as Advanced Directives on the protection of patient/family rights Practitioners manage program resources (financial, human, material or informational) to facilitate safe, effective, and efficient care within the medical center or VISN. Maintain knowledge of current techniques, trends and professional issues. Ensure practice and program policies/procedures meet standards by using established professional, community or regulatory standards as benchmarks to evaluate programs and/or service activities within the service line or medical center. Function as clinical consultants within the service line and other service lines within the medical center or VISN.
