The Certified Nurse Anesthetist will perform both the duties of a staff CRNA and an Educator-Coordinator.

Duties

The incumbent in this position will perform general CRNA duties and responsibilities 60% of the time. These duties include the following: Completes a patient evaluation and progress notes as Spokane VAMC provider bylaws, regulations and policies require. Write pre-op anesthesia order. Completes pre-anesthesia assessment, immediately prior to the initiation of anesthetic. Determines that patient is candidate for planned choice of anesthesia after completing pre-anesthesia assessment and before the administration of anesthesia. Documents findings according to hospital policy. Induce general, regional, or monitored anesthesia care, or combinations thereof, control emergence & extubation of patients. Maintain anesthesia at required levels. Support life functions during the administrating of anesthesia, including induction and intubation. Recognize and take appropriate action for abnormal patient responses to anesthesia or to any adjunctive medicine or other form of therapy. Provide professional observation and resuscitative care until patient has regained control of vital functions. Complete post assessments and progress notes and any follow-up orders needed for patient care. Emergent code/floor intubations, Run ACLS Code as certified until physician arrives. Act as resource for staff education. Participate in administrative and committee functions as requested by Supervisory Anesthesiologist, Chief of Surgical Service and/or Chief of Staff. On call during off tour hours and weekends. Availability is one hour for inpatient and surgery. Special Procedures: Spinal Anesthesia single injections Epidural Anesthesia single injection, lumbar or thoracic Spinal Anesthesia continuous Epidural Anesthesia continuous, lumbar, or thoracic Combined spinal-epidural anesthesia Brachial Plexus Block Stellate ganglion block Intravenous block of upper or lower extremity (Brier Block) Other block procedures- such as retrobulbar ophthalmic blocks or additional blocks as privileges are granted by medical staff Insertion of arterial catheter Insertion of central venous catheter to include peripheral route, internal or external jugular, and. subclavian route Insertion of pulmonary artery catheter The incumbent in this position will perform Educator-Coordinator duties 40% of the time. These duties include the following: Functions as the primary instructor trainer for the Station providing direct clinical and instructional supervision to full time Nurse Anesthesiology Doctoral students. Ability to serve as interdepartmental liaison, interfacing with other departments such as nursing, surgery, PACU, outpatient surgery, admissions, administration, laboratory, or pharmacy to incorporate the expertise of all disciplines in a comprehensive, integrated approach to care. Knowledge to provide advice and assistance to other CRNAs in unusually difficult of complex cases Ability to perform clinical and didactic education, commitment to in-service training for staff and facility continuing education to provide accurate information to population serviced Ability to provide feedback to students, faculty and the Program or Assistant Program Director in a timely fashion. Skills for the development, implementation, evaluation and on-going updating of clinical curriculum and evaluation. Ability to administer, evaluate and provide sound statistical research, which may promote clinical and didactic evolution and expertise in the field of anesthesia Knowledge to implement educational strategies to meet changes, clinically, academically, and professionally, relate to nurse anesthesia. May chair or co-chair graduate level research to assist students in researching, reviewing/approving research proposal and finished product Knowledge to support and prepare for the program's accreditation process by the COA of Nurse Anesthesia. Ability to facilitate communication between students, faculty, and other site coordinators to oversee/manage the clinical education of students. Qualifications

Advanced Practice Nurse (Certified Nurse Anesthetist) Basic Requirements English Language Proficiency. In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education. A masters or doctoral degree in nurse anesthesia. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Nursing education must be from a school of professional nursing anesthesia accredited by Council on Accreditation of Nurse Anesthesia Education Programs (COA) at the time the program was completed by the applicant. Licensure and Certification. The following licensure and certification requirements are considered conditions of employment. License. All APN (CNAs) must maintain an active, current, full and unrestricted license to practice as a CNA or equivalent, in a State, Territory or Commonwealth of the United States or in the District of Columbia. Basic Certification. All APN (CNAs) must maintain full and current certification by the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA) or an equivalent Nurse Anesthetist certification recognized by the COA. NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (CNA) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Education, Experience and Knowledge, Skills and Abilities must be met for Advanced Practice Nurse (Certified Nurse Anesthetist) IV, Education Coordinator: Education and Experience Requirement: Meets basic education requirement for an APN (CNA) AND Four years of APN (CNA) progressively responsible assignments in clinical, administrative, education and/or research related to CNA practice, one of which is specialized experience equivalent to the APN (CNA) III grade level. Demonstrated Knowledge, Skills, and Abilities: Must have the ability to perform long and short-term planning goals for their anesthesia department. The APN (CNA), Educator Coordinator functions may include, but are not limited to, the following knowledge, skills, and abilities: Ability to develop, implement and evaluate a clinical curriculum and provide feedback to students, faculty, and leadership in a timely fashion. Ability to provide mentorship regarding investigating, evaluating, and translating evolving research findings for implementation into clinical practice, quality improvement, policy development, system transformation and enhancing efficiency, effectiveness, and patient outcomes. Ability to conduct, collaborate and assist with approved research and scholarly projects that enhance students and providers' knowledge and expertise in the profession and the missions of VA. May chair or co-chair graduate level research and/or scholarly projects to assist students in meeting program requirements. This may include mentorship in research/scholarly project development, literature search/review and evaluation, approval processes, scholarly writing, implementation, and completion/defense. Skill in investigating and advancing education related to current anesthesia practice for healthcare providers both within and outside the anesthesia specialty. Knowledge to implement educational strategies to meet changes clinically, academically, and professionally, related to anesthesia. Ability to perform clinical and didactic teaching, in-service training for staff and facility continuing education to provide appropriate information to the population served. Ability to serve as interdepartmental liaison to incorporate multidisciplinary expertise in a comprehensive, integrated approach to care. Skill in providing guidance and assistance in unusually difficult or complex anesthetic management. Knowledge to provide support for preparation of an educational program's accreditation process (i.e., by the COA).
