The Supervisory Social Worker plans, develops, implements and oversees all components of seamless transition and services provided to Military to VA (M2VA) Care Management Program, Patient Aligned Care Teams (PACT), and Oncology Veterans. Work requires the exercise of mature professional judgment and flexible use of administrative, leadership, and social work skills.
Duties
Duties and responsibilities include, but are not limited to:
Responsible for the programmatic, professional and administrative management of the assigned staff in PACT, Oncology, and M2VA Care Management Program, including Social Workers, Nurses, Transition Patient Advocates and administrative support.
Oversees the Transition Patient Advocates, including monitoring the caseload and the advocacy services provided to service members and/or Veterans in the assigned programs.
Responsible for the oversight and implementation of the Patient Aligned Care Team Social Work Case Management Practice Model with the assigned staff and providing accountability for program effectiveness and modification of case management service patterns to enhance customer satisfaction.
Identifies gaps in services and develops a plan, in consultation with facility staff, that will make available the best possible resources in meeting program needs. Collaborates and negotiates with multiple disciplines and service lines to accomplish this.
Manages the daily operations, planning, developing, establishing and implementing policies and procedures to ensure compliance with the Joint Commission, Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards as needed.
Establishes performance and outcome standards for the facility PACT, Oncology and M2VA Programs that promote quality and efficiency of services in coordination with the overall goals of the medical center and those of the VISN and VA Central Office to include data collection, monitoring, and analysis with established goals and outcomes.
Oversees the system to identify patients served by the facility and those in the catchment area not currently seen. This may involve use of a OEF/OIF Registry, Patient Treatment File searches, the Care Management Tracking and Reporting Application (CMTRA), Caregiver Application Tracking System (CAT), Veterans Health Information System and Technology Architecture (VistA) and Computerized Patient Record System (CPRS) records, Northeast Program Evaluation Center (NEPEC) data reports and/or Classification reports, Patient Data Exchange, Network Health Exchange, contacts with the Veterans Integrated Service Network (VISN) and VA Central Office, and contacts in the community.
Serves as the facility Social Work point of contact for all inquiries regarding Service Members seeking to establish care.
Provides education to M2VA, Oncology and PACT service members and Veterans on VHA health care services available at the facility and about services available within the VISN and in the local community.
Ensures that the M2VA Program is interdisciplinary and fully integrated with the inpatient and outpatient medical programs.
Coordinates psychosocial evaluation, assessment, and periodic reassessments of patients served, which provides a comprehensive database to identify psychological, social, and vocational needs and the appropriate treatment and services to be provided. Ensures that all M2VA and PACT service members and Veterans are screened for the need for care management services and that those in need of nurse and/or social worker Case Managers are assigned accordingly, particularly those who are severely injured or ill.
Maintains a current network of referral resources to include substance abuse treatment, outpatient medical and/or psychiatric care, vocational rehabilitation, etc. and with VSOs.
Establishes and maintains an ongoing education program for patients, community agencies, students, and staff. Provides consultation to other staff on the special needs of PACT, Oncology and M2VA service members and Veterans.
Work Schedule: Monday through Friday, 8:00am - 4:30pm

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-12 Supervisory Social Worker candidates must have the following:
Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
Ability to ensure provision of clinical social work services.
Ability to provide staff training and development.
Skill in interpersonal relationships in dealing with employees, team leaders, and managers.

The full performance level of this vacancy is GS-12.
Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
