This position functions in Pharmacy Service to support timely access to quality health care for our Veterans at the Gulf Coast Veterans Health Care System (GCVHCS). The primary responsibility of the Patient Aligned Care Team (PACT) Clinical Pharmacy Specialist (CPS) is the evaluation of medication therapy through direct patient care assessment.
Duties:
Responsible for outpatient ambulatory care pharmacy activities involving highly innovative methods of health care delivery in clinic. Includes direct patient care services through individual visits, group visits, telephone visits, video conference visits, chart review, and use of secure messaging.
Participates in a patient-focused, team-based multi-disciplinary model of patient centered care to promote positive patient outcomes through direct and indirect interaction with patients and providers.
Serves as a mid-level provider by directing therapy; ordering lab tests; performing physical assessments; and initiating, modifying and discontinuing medications as defined within the Clinical Pharmacy Specialist's scope of practice.
Conducts medication therapy management clinics and group forums (e.g. diabetes, smoking cessation, gout, pain, dyslipidemia, etc.).
Evaluates appropriateness of pharmacologic therapy at clinic visits and as the level of care changes. Develops appropriate therapeutic monitoring plans and documents the activities in the medical record via electronic progress notes.
Provides medication reconciliation and patient medication counseling verifying and documenting patient understanding of the medication, proper medication use, expected outcomes, how to obtain refills of medication and how the current prescription will be processed.
Performs panel management through various dashboards and resources to identify patients that may benefit from a PACT CPS visit to align with facility initiatives and goals including workload requirements.
Reports actual and/or potential medication errors or adverse events and participate in quality improvement initiatives to promote patient safety. Actively participates in the VA Adverse Drug Reaction (VA ADR) reporting program.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT)
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Grade Determinations: Clinical Pharmacy Specialist GS-13
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Pharmacists assigned to this position must demonstrate the following KSAs:
1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
2. Expert understanding of regulatory and quality standards for their program area.
3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time .
Grandfathering Provision: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Preferred Experience:
1. Completion of a PGY2 ambulatory care pharmacy residency or PGY1 general pharmacy practice residency with a significant component focused in ambulatory care and board certification in pharmacy specialties i.e., Board Certified Pharmacotherapy Specialist (BCPS} and/or Board Certified Ambulatory Care Pharmacist (BCACP}
2. Experience working in a Veterans Affairs PACT Clinic

The full performance level of this vacancy is GS-13.
