This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Duties

The Compounded Sterile Preparations (CSP) Pharmacy Supervisor must be a licensed pharmacist responsible for the comprehensive management of the Pharmacy Service's Sterile and Non-sterile compounding programs as well oversight of hazardous drug management for the Miami VA Healthcare System. The Pharmacy Supervisor will also supervise the medication safety program which includes a high level of oversight to ensure compliance with hospital accreditation standards. The position oversees activities where the Pharmacy Supervisor has full and final technical and/or administrative authority over the work, which requires coordination and integration of work assignments with other service lines. The duties and responsibilities of the Pharmacy Supervisor may include, but are not limited to, any of the following: Maintains environmental control of the sterile compounding areas. Possess knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. Responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy. Applies standards relating to all aspects of distribution and control of scheduled and non-scheduled drugs. Responsible for all aspects of ensuring medication safety, performance improvement, and regulatory compliance. Develops policies and procedures to ensure that hazardous drugs (HDs) that are transported, stored, and handled by other health care professionals are appropriately labeled to alert staff to handle the HDs as required by USP . Provides oversight for medication error reporting within pharmacy service. Represents pharmacy on applicable patient safety task groups. Manage the adverse drug reaction reporting system (VA ADERS) Supervises pharmacists and technicians working within the pharmacy section. Tracks trending data and prepares reports. Establishes and implements quality assurance monitors. Works with supervisors to resolve issues or implement system changes to prevent medication errors. Educates staff on medication error prevention and system changes. Submits reports to the FDA MedWatch program and other entities as required. Ensures that Pharmacy Service is compliant with the Joint Commission continuous readiness program. Provides medication counseling to patients. Completes medication histories including patient interviews. Participates in the medical center medication utilization evaluation (MUE) program. Recommends formulary alternatives and completes non-formulary consults as assigned. Performs all other duties as assigned. Education: (Transcripts Submitted) (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. Licensure (Documentation Submitted): Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence, or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration, or certification. Grade Determinations: In addition to the above basic requirements, candidates must also meet the following grade requirements: GS-13 Grade Requirements: Experience: (1) In addition to the GS-12 requirements, must have one (1) year of experience equivalent to the next lower grade level (GS-12). OR (2) Completion of one (1) year Post Pharm D. ASHP accredited Residency Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. Pharmacy Supervisor. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area.
