The Nurse IV: Skilled Home Care Program Manager (PM) executes leadership that is characterized by substantial and continuous responsibility and accountability for Skilled Home Care programs and services within the Office of Geriatrics and Extended Care's (GEC) Purchased Long Term Services and Supports. This position is covered by the National Nurses Organizing Committee, National Nurses United (NNOC/NNU) bargaining unit.

Duties

This position may be filled as virtual (i.e., the employee may work at a VA facility or VA-leased space other than the facility that is hiring the employee and may telework) and is authorized for telework up to 50%. VA supports the use of telework and filling virtually to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Telework eligibility and potential duty location may be discussed during the interview process. Major Duties include, but are not limited to: Functions as an advisor, administrator, educator, consultant, and mentor who possesses a wide range of experience and leadership skills. Provides leadership and oversight for VACO clinical and administrative operations as directed by the National Program Manager, Purchased Long Term Services and Supports. Supports the integration and collaboration of nursing with other professional disciplines and functional areas in the mutual achievement of Veteran-driven and organizational goals. Relates effectively with VACO and program healthcare executives, and the community as well as program and service leaders at the network and facility levels. Assists the National Program Manager with healthcare operations and the functions and activities expected of them. Contributes ideas and recommendations for the establishment and revision of standards of care, policies, and objectives for the enhancement of Veteran care. Provides program management of the Skilled Home Care programs and acts to improve the quality and oversight of these programs. Coordinating activities related to the planning, development, and execution of all related skilled home care programs, coordinating the general program support work such as collection, compilation, research, and/or tracking of data and program information. Advises and provides technical and clinical expertise on overall operations and functions for all Veterans Heath Administration (VHA) Skilled Home Care program activities and collaborates across numerous offices and locations. Performs high level administrative or technical program work in support of the program, office, or of the organization. Provides leadership in the development, enhancement, and improvement of national standards, policies, and guidelines to improve the effectiveness, quality, and oversight of Skilled Home Care programs and for Veterans who require these services. Supports national and VISN level activities and field facilities by providing clinical program and policy guidance and assistance with the development of interdisciplinary approaches to the accomplishment of strategic plans and initiatives related to the Skilled Home Care programs. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. NOTE: This applies to past, current, and upcoming VA Health Professions Trainees (HPT) only. VA HPT is an individual appointed to temporary positions in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or admitted to the VA Health Professional Scholarship Program. HPTs can be either VA-paid or Without Compensation (WOC). Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.) NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Scope, Education and Dimension criteria must be met to qualify for Nurse IV: Scope: Executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines and influence organizational mission and health care. Education: Master's degree in Nursing (MSN) and 4 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR Master's degree in a *related field with a BSN and 4 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR a Doctoral degree in nursing and 3 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR a Doctoral degree in a *related field with a BSN and 3 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. *Note: Foreign education programs/degrees are not creditable as related degrees. Physical Requirements: The incumbent must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands with reasonable accommodation, if necessary, without endangering the health and safety of the Nurse or others. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. Preferred Experience: Geriatric and Extended Care Purchased Long Term Services and Supports, specifically Skilled Home Care (Skilled Home Health Care, Home Infusion, Hospice, Palliative Care, Bowel and Bladder Care, Program for All-Inclusive Care of the Elderly). Office of Integrated Veteran Care (IVC) policies and procedures. Working in a role that demonstrates the ability to develop and implement Federal regulations, national policies, or enterprise-wide processes/procedures.
