Gastroenterology (GI) Medical Instrument Technicians (MIT) assists practitioners with endoscopic medical procedures, such as colonoscopy , endoscopic ultrasound, esophagogastroduodenoscopy (EGD), and endoscopic retrograde cholangiopancreatography (ERCP). GI MITs ensure proper performance of equipment, set up equipment, and reprocess endoscopic reusable medical equipment (RME).

Duties

FUNCTIONS or SCOPE OF ASSIGNED DUTIES: 1. Advanced knowledge of complex gastroenterology and pulmonary procedures such as mucosal resection and ablation, fine needle aspiration, and double balloon colonoscopy, using a variety of highly complex ancillary equipment, which encompasses 40% of their assigned duties. 2. Ability to plan and assist in establishment of completely integrated treatment and examination programs. 3. Ability to evaluate new products and equipment and make recommendations concerning developments which would improve clinical operations. 4. Ability to support local leadership in management of fiscal matters, forecast resource equipment needs, and submit required acquisition documents for routing and approvals. 5. Ability to orient/precept lower grade MITs in work to be performed, assist in peer review, selection of staff, and provide recommendations for awards, advancements, and, when appropriate, disciplinary actions. 6. Assists physicians directly with diagnostic and therapeutic procedures such as Colonoscopy, Sigmoidoscopy, PEG placement, endoscopic mucosal resection (EMR), esophagogastroduodenoscopy (EGD), endoscopic retrograde cholangiopancreatography (ERCP) with spy glass, ablation procedures, mucosal resection procedures, fine needle aspiration procedures, capsule studies, endoscopic ultrasound (EUS), bronchoscopy and endobronchial ultrasound (EBUS), ION robotic pulmonary procedures, and advanced biopsy techniques. 7. Assures sterility of procedural fields and operational safety of procedure-related medical devices, instrumentation, and reusable medical equipment (RME). 8. Assists with interventions to protect patients from hospital acquired infections, participates in the Time-Out process, adheres to implant, and specimen handling and management policies and procedures, recognizes changes in patient care situations, and assists with initiation of appropriate interventions and/or emergency responses for patient safety. 9. Uses a team approach to help identify and resolve patient care problems with measurable outcomes. 10. Independently performs procedure set-up, assists with biopsy procurement, snare polypectomy, esophageal, pyloric, colonic dilation; treatment of achalasia with pneumatic balloon dilation; variceal banding; injection of vasoactive medication, and use of bicap/bipolar electrocautery; argon plasma coagulation for treatment of bleeding, deployment of surgical resolution clips to visible vessels; foreign object removal using Roth net basket or 3-prong alligator forceps, and injection of sites with tattoo spot marker. 11. Conduct trials on new equipment and coordinate major repairs or installation of equipment with vendors and other facility departments; arrange vendor training and other educational in services for the endoscopy staff. 12. Assures sterility of procedural fields, therapeutic instruments, medical devices, and related reusable medical equipment (RME). 13. Ability to collaborate, communicate, and demonstrate customer service skills and interpersonal skills with all healthcare professionals to ensure quality and continuity of care. 14. Ability to use basic computer systems and programs to record patient and endoscopic data, adhering to patient privacy and information security policies. 15. Assists interdisciplinary procedural team members in protecting patients from injury caused by therapeutic instruments, medical devices, related equipment, and extraneous objects. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-8 (Positions at this grade may be lead or non-supervisory with specialized advanced knowledge and assignments.) Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, and abilities related to the duties of the position to be filled. This would be experience which provided knowledge of advanced specialized gastroenterology procedures. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Demonstrated Knowledge, Skills, and Abilities. KSAs 1, 2, 3, and 4 are appropriate for all positions at this level. KSA 5 is to be used as appropriate for the specific assignment. 1. Knowledge of complex gastroenterology procedures, such as ERCP with spy glass, ablation procedures, mucosal resection procedures, fine needle aspiration procedures and capsule studies. 2. Ability to provide briefings and orientations to hospital staff including physicians. 3. Knowledge of regulatory and advisory agencies such as Joint Commission, Occupational Safety and Health Administration (OSHA), Society of Gastroenterology Nurses and Associates (SGNA), American Society for Gastrointestinal Endoscopy (ASGE), and applicable laws such as the Health Insurance Portability and Accountability Act (HIPPA). 4. Knowledge of medical terminology related to GI and biliary systems used to identify pathology or for specimen identification and procedure documentation and knowledge of advanced anatomy and physiology to recognize obvious abnormalities during procedures. *5. Ability to provide staff development and training. Preferred Experience: 2-3 years of MIT experience assisting with General and Advanced GI and Pulmonary procedures. Physical Requirements: The population of male and female Veterans served ranges from adult, age 18, to the elderly adult, age 65 and over. VA San Diego Health Care System is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, and twisting. Transferring patients and objects are required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations and/or during operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
