The Licensed Vocational Nurse in the Primary Care is a direct care provider whose scope of practice includes providing direct nursing care to patients in the ambulatory care setting at the VA Central California Health Care System, Fresno of Primary Care Service. Incumbent practices in the outpatient care setting under the direction and supervision of the Clinic Nurse Manager.

Duties

Incumbent should have or quickly acquire the ability to provide effective education to patients and/or family members in relation to common disease processes, medication and/or treatment regimes. Acquire the ability to completely & confidently prepare, administer and appropriately document actions taken specific to commonly prescribed oral, topical, intradermal, subcutaneous, and/or intramuscular mediations, immunizations or skin tests as permitted by approved local facility policies and procedures. Acquire the ability to astutely observe and document a patient's response to medications administered and report any change in patients' condition to RN or MD/DO, as appropriate. Acquire the ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and/or pulmonary arrest. Provide effective education to patients and/or family members in relation to common disease processes, medication and/or treatment regimes. Knowledge and skill sufficient to completely & confidently prepare, administer and appropriately document actions taken specific to commonly prescribed oral, topical, intradermal, subcutaneous, and/or intramuscular mediations, immunizations or skin tests as permitted by approved local facility policies and procedures. Ability to astutely observe and document a patient's response to medications administered and report any change in patients' condition to RN or MD/DO, as appropriate. Adheres to standards of practice and safe technique in preparation and administration of medications. Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed and when appropriate. Based on the patient's goals, provides patient, family and caregiver health education, to include self management, prevention, and wellness. Education. Graduate of a school of practical or vocational nursing approved by the appropriate State [ ] agency and/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Grade Determinations: In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade.
