A disaster can strike at any time - an earthquake, tornado, hurricane, or global pandemic can leave our Veterans, families, and communities in desperate need of help. That's why the U.S. Department of Veterans Affairs (VA) is creating Clinical Deployment Teams (CDTs) to stand ready to assist. We're looking for clinical personnel to serve on these teams and help transform how we provide health care services in times of great need.
Duties
The incumbent is located at one of the sites of care for the VA Central California Health Care System, including the main Fresno campus, the Community-Based Outpatient Clinics (CBOCs) in Oakhurst, Merced and Tulare. Future locations may be added or deleted based on organizational need. The incumbent supports the Veterans Health Administration (VHA) expanded provision of patient care to various institutional and non-institutional settings, such as the Department of Veterans Affairs Medical Centers (VAMCs), Community-Based Outpatient Clinics (CBOCs), State Veteran Homes, Vet Centers, homeless shelters, Indian Health Services (IHS), Department of Defense (DOD) and other affiliated organizations by using innovative telecommunication technologies to enhance care coordination, access to care, and to routinely and proactively manage varied health states to prevent healthcare crises. These programs include Clinical Video Telehealth (CVT), MyHealtheVet (MHV), Home Telehealth (HT or CCHT), and Store and Forward (SF).
LPN/LVN responsibilities include:
The work involves the independent performance of the full range of functions using a variety of conventional techniques and procedures as described below.
The work involves a variety of patients of diverse cultural, spiritual, and education backgrounds.
The work directly affects the overall well-being of patients served.
The work involves the independent involvement of scheduled appointments following accepted facility practices and national scheduling directives.
The employee must be able to identify materials and techniques used with each patient.
Guidelines include operating policies and practices of the facility including those involving telehealth; accepted materials, procedures and techniques associated with each piece of equipment for telehealth; and other facility publications including memoranda of understanding, service agreements, and telehealth plans.
Equipment for telehealth includes items such as clinical video teleconferencing (CVT) carts, teleretinal imaging cameras, MOVI cameras, telederm cameras, cardiac device interrogators, and similar type of equipment used to capture images, voice, video, or electrical signals and transmit them between the patient and a remotely located provider.
The employee uses judgment in selecting and adapting techniques, procedures, and materials most appropriate for the specific condition of each patient in an independent manner, often serving as the only Telehealth personnel on site.
Materials may include components of the telehealth equipment such as stethoscopes, transducers, and EKG recording devices or educational materials for the providers, clinic staff, or patients.
Educational material address the process of telehealth, required follow up, and roles and responsibilities of the remote providers (readers) and the local provider and patient. Situations requiring deviations from the guidelines are referred to the supervisor.
Work Schedule: Tour of Duty 8am to 4:30 pm

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Education - Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment OR granted a license by a jurisdiction that does not require graduation from an approved school AND have at least one year of successful practice as an LPN/ LVN.
Licensure - Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia.
Grade Determination: In addition to the basic requirements stated above, the following qualification criteria must be met. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
GS-07: The GS-07 grade level is for select, complex LPN/LVN positions, as established by each health care facility. To be advanced, employees must meet all of the following standards, provided the standard is part of their assignment. Completion of at least one (1) year of additional qualifying experience at the GS-06 level or equivalent, fully meeting all of the performance requirements for the GS-06 LPN/LVN.
Knowledge, Skills, and Abilities (KSAs) must be met:
Knowledge of performing telehealth imaging, including tele retinal screening, tele dermatology and tele wound care, as identified, and referred by the VA provider.
Knowledge of providing technical and administrative support for telehealth programs
Knowledge of identifying and scheduling patients for digital imaging
Knowledge of collecting appropriate patient demographics and pertinent historical information to populate the imager template if not completed by clerical personnel.
Knowledge of providing basic information and education to patients related to the imaging process (before, during, and after as needed).
Knowledge of exercising timely and appropriate action based on the consult report from the readers.
Knowledge of serving as a liaison between image readers, referring providers, specialty care providers, patients, and other VA personnel.
Knowledge of mechanics and functioning of all associated telehealth equipment (noted above) in the imaging clinic, and cleans, performs routine minor maintenance, and requests service for all ophthalmic and computer equipment.
Preferred Experience and Certifications
2-3 years of experience as in LPN/LVN experience
Basic Life Support (BLS)

NOTE: Current GS-0620 LPN/LVN employee of the Veterans Health Administration (VHA) who were converted to an appointment under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, or who was converted to an appointment under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986 are considered to have met the Education requirement. The waiver does not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions.
Physical Requirements: Eligible applicants must be physically and mentally able to perform efficiently the essential functions of the position, with or without reasonable accommodation, without hazard to themselves or others. Depending on the essential duties of a specific position, usable vision, color vision, hearing or speech may be required. However, in most cases, a specific physical condition or impairment of a specific function may be compensated for by the satisfactory use of a prosthesis or mechanical aid.
