Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
Duties
The Nurse Manager, Primary Care (NM, PC) serves as a first level supervisor who functions under the direction and guidance of the Chief Nurse, Primary Care. S/he enhances the development and implementation of the Primary Care's Patient Aligned Care Team (PACT) Program by working in partnership with their site leadership as well as with Primary Care Leadership at the Medical Center. The NM, PC demonstrate leadership, experience and creative approaches to work independently optimizing patient care, patient safety and operational efficiency for patients receiving care within the Primary Care Program. The NM will demonstrate expertise in leading and organizing care delivery, staff accountability for practice, promotes staff engagement in decision making and guiding and supporting staff growth and development. S/he utilizes evidence-based practice to develop and evaluate program and service activities that impacts positive patient outcomes within Nursing Service and Primary Care. The NM, PC primary responsibility is to lead the delivery of the PACT model of care being provided by staff under his/her supervision by reviewing, assessing, and measuring outcomes related to the processes involved in preventive and disease management that is patient driven and utilizes a team-based approach. This position is aligned under Patient Care Services, but work collaborative with Primary Care Leadership to meet the mission and vision of the Medical Center. This position requires cross-coverage at the Primary Clinics and CBOC's in absence of another Nurse Manager.
Nurse Manager (NM) focus is on individual growth and development in practice with increased levels of self-direction. This nurse brings principles and theory with limited application of critical thinking skills. The NM is the designated first-line supervisory authority and is responsible and accountable for the operation of their assigned units and provides coverage for other NMs as needed. The NM is responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance. They function as a role model, educator, practitioner, and supervisor providing leadership to the team. They enhance care outcomes by collaborating with members of the interdisciplinary team to remove barriers and implement new evidence-based cutting-edge programs/processes.
The NM, PC provides 24-hour management, supervision, and leadership for a Primary Care Nursing at the designed clinic or CBOC. This includes but is not limited to resource utilization (human, supplies/equipment), patient flow, performance improvement activities, and staff and program evaluations. is responsible for all issues that affect nursing including standards of practice, credentialing, training, functional statements, position descriptions, timely proficiencies and performance appraisals, and ensures compliance with all policies, procedures and protocols. The NM, PC ensures appropriate orientation, education, competence assessment, and continuing professional development of nursing personal supervised. The NM, PC possesses the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting, which will enable effective implementation of health promotion and prevention practices, management of acute and chronic illnesses, and attainment of optimal levels of functioning and/or support of patients and significant others in accordance with the PACT principles. S/he keeps staff informed of unit and executive-level activities in a timely matter, and through monthly staff meetings. The NM, PC will communicate and/or prepare information in a timely matter regarding emergencies regarding unusual occurrences, or issues that impact nursing practice in Nursing and Primary Care. The NM, PC is responsible for staffing and scheduling of nursing staff and ensures that assignments of staff reflect their appropriate competence and utilization. S/he ensures that nursing practice is consistent with external standards such as Joint Commission, Office of Inspector General and internal standards such as VA Performance Measures to include various reimbursement agencies.

Qualifications
Basic Requirements:
English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The position requires potentially long periods of combined walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent must be a mature, flexible, sensible individual capable or working effectively in stressful situation, able to shift priorities based on needs of the organization. The incumbent who provides off-site training may be required to travel occasionally. This position potentially requires flexibility in schedule and assignments.
