Incumbent works in the Home Telehealth Program (HT) office within the Primary Care Service Line at the Columbia VA Health Care System. The HT Program coordinates care for a wide range of eligible Veterans. The nature of work performed is to assist HT staff in supporting and meeting the directives of care coordination and managing chronic medical and mental health diseases to prevent healthcare crises. The incumbent will provide administrative support regarding the HT program objectives.
Duties
Incumbent exercises judgment and reason and relieves Supervisor of as many administrative duties as possible. The HT Program Support Assistant performs a variety of clerical and administrative support duties for the HT Program and performs office data support using multiple automated programs and software such as SharePoint, databases, spreadsheets, word documents and graphics in support of the clerical and administrative work.
Duties include but may not be limited to the following:
Handle and collate data for patients enrolled in the Home Telehealth(HT )Program.
Serve as a point of contact for Veterans, medical center staff, community agencies, and various others who contact this office.
Assist with any future proposals/projects that are implemented, distributes, and balances the workload among Care Coordinators, assures timely accomplishment of the assigned workload, helps with enrollment, disenrollment and troubleshooting non-responding activities on telehealth equipment and documenting these as appropriate.
Collect, compile and/or track statistical information and patient census report for the support of program operations and to access status or outcomes in relation to established performance measures, goals, and objectives; receive and process documents and reports in accordance with established procedures; use software applications to draw information from a wide variety of sources in order to prepare reports; and assist in the planning, review, and reporting of data/statistical results of program studies.
Receive and ascertain the nature of the calls or visits and directs telephone calls, messages, and patients/visitors.

Qualifications

Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05 level. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:

Collect, compile and/or track statistical information and patient census report for the support of program operations and to access status or outcomes in relation to established performance measures, goals, and objectives;
Receive and process documents and report in accordance with established procedures; and

Preferred Experience: Two years of administrative duties with medical terminology experience.

Physical Requirements: The work is sedentary. Typically the employee sits comfortably to do the work. However, there may be some walking, standing, bending, carrying of light items such as books, papers. No special physical is required to perform this work.
