The Social Work Program Coordinator, locally titled Caregiver Support Program (CSP) Manager will supervise professional and non-professional staff including support staff. They will have significant program management responsibilities. The social work program coordinator is responsible for the professional, clinical, and administrative management of the Caregiver Support Program but can be assigned to all social work program areas to provide professional, independent social work services.

Duties

Major duties include: Supervises staff assigned to Caregiver Support Program (CSP) as outlined in VHA Directive 11152(1), Caregiver Support Program (CSP). Coordinates program implementation with multiple clinical services across all campus locations within the VA facility, including but not limited to: nursing; physical, occupational, and speech therapists; mental health and primary care providers. Serves as a consultant to other VA professionals and administrators on program specific activities. Serves as a liaison to Veterans Integrated Service Network Program (VISN) CSP Program and the VA Central Office (VACO) Program. Serves as the VA CSP point of contact for all inquiries and outreach requests regarding eligible Program of Comprehensive Assistance for Family Caregivers (PCAFC) and Program of General Caregiver Support Services (PGCSS) caregivers. Identifies unmet needs of the caregiver population, their families/significant others and caregivers. Works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions-participating in national, regional, and VISN operational calls; contributing to national leadership council committees; and implementing cultural transformation initiatives. Develops and implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services. Establishes guidelines and performance expectations for staff, clearly communicated through the formal employee performance management system. Provides informal and formal feedback and evaluating employee performance, oversees time keeping and manages recruitment and retention activities. Addresses and resolves issues in conjunction with departments to include but not limited to, Human Resources, Equal Employment Opportunity (EEO), Employee Relations/ Labor Relations, Reasonable Accommodations, Payroll, Time Keeping, etc. in all functions ranging from hiring actions, grievances, employee complaints, review disciplinary cases (i.e., those proposing suspensions and removals) and participate in fact finding. Monitors performance measures at the local, VISN, and Central Office levels; providing responses to congressional inquiries; and briefings to providers, VA leadership, community organizations and other internal and external stakeholders. Develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. Develops a system to identify family caregivers served by the VA facility including caregivers in the catchment area not enrolled in CSP. Plans, organizes and coordinates administrative, operational, clinical and personnel activities for the CSP program. Provides oversight of facility CSP program fiscal matters of the functions supervised (which would include fund controls, contracts, and equipment expenditures), forecasts resource and equipment needs, and administers the allocated budget. Manages all aspects of the PCAFC and the PGCSS program to ensure eligible caregivers have awareness and access to the national Caregiver Support Line (CSL), VHA's Peer Support Mentoring Program, self-care and resiliency building trainings (e.g. Building Better Caregivers), and personal care service skill development trainings (e.g., VHA Caregiver Core Curriculum) regardless of era of service. Provide subject matter consultation to colleagues and students on the psychosocial treatment of Veterans offering professional opinions based on experience, expertise, and role modeling effective social work practice skills. Provides training, consultation and education to VA staff as well as community agencies on VA policies concerning CSP. Provides training to VA staff and community stakeholders on the role of social work, social work practices, psychosocial resources at the VA and in the community, and research pertaining to their scope of practice. Participates and leads/co-leads project activities and process improvement projects in the hospital and will be expected to complete the Yellow Belt process improvement within two years of the assignment to position at SAVAHCS. Develops and monitors standards of clinical practice/standards including development of practice guidelines, competencies, manuals, and guides to professional practice using data, research, and evidence-based practice. Develops consistent standard operating procedures required to accomplish clinical care interventions and ensures compliance. Serves the Medical Center and social work profession in a variety of capacities, such as committee, unit councils, workgroups, special assignments, or other duties as assigned that are necessary to meet program goals and VA Mission. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Proficient in spoken and written English as required by 38 U.S.C. 7403(f). Hold a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in social work. Licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Social Worker (Program Coordinator) Licensure: Must be licensed or certified at the advanced practice level, and able to provide supervision for licensure. GS-13 ASWB Examination: Must have completed and passed the Association of Social Work Boards (ASWB) advanced generalist or clinical examination. GS-13 Social Worker (Program Coordinator) Experience: Must possess one year of experience equivalent to the GS-12 grade level. Examples of specialized experience include, but are not limited to: Serving as the sole practitioner in a specialty program and typically provide direct patient care services in the program area; oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics; may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery; provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. GS-13 Social Worker (Program Coordinator) Knowledge, Skills and Abilities: In addition to meeting the licensure and experience for the GS-13 level, you must possess the following Knowledge, Skills and Abilities: Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Physical Requirements: Physical demands of the work include sitting, standing, walking, standing, driving, pushing equipment and wheelchairs, lifting, bending, stooping, climbing stairs, and carrying light items such as papers, books, or other supplies. The incumbent uses a computer and standard office equipment to complete tasks. The emotional demands can be stressful in working with the Veteran/patient population and their families/caregivers. A level of self awareness and emotional intelligence is essential. Incumbent must have a valid Drivers License and must be able to drive a vehicle to fulfill job functions as this position requires outreach activities.
