This position is located within the Claremore Service Unit, under the supervision of the Outpatient Nurse Manager or designee. As the RN-Care Manager (RN-CM), the incumbent is responsible for all aspects of Care Management for empaneled patients assigned to their designated care team, assisting in the development of the Plan of Care, ensuring that the plan is implemented in a timely basis, and identifying patient needs.

Collaborate with primary care providers, clinic nurses, public health nursing, DM case management and social services to ensure that the patient's care needs are completed in a timely and cost effective manner to maximize each visit. The care manager is responsible to ensure that health care maintenance is current on empaneled patients. Coordinates clinic visits for assigned patients with other members of the multidisciplinary team in order to optimize care.
Serves as primary contact/clinical liaison between referral hospitals, treatment facilities, and patients and their families to ensure delivery of effective and appropriate treatment in accordance with established plan of care. Facilitates, schedules, and/or participates in a variety of patient care coordination activities to ensure patient and/or family health care needs are met timely. Proactively identifies and coordinates care within alternative resources as needed.
Contacts patients for follow-up diagnostic tests, referrals, appointments, or other medical issues as appropriate in collaboration with the provider. Participates in specific clinical initiatives focused on improving patient 's medical self-management to include, but not limited to, compliance with scheduled clinic visits, medication regimens, and scheduled appointments at outside medical facilities.
Interacts with the patient/family/caregiver and primary care provider to develop and discuss the patient's plan of care, understanding of disease processes, and coordination of services based on patient's clinical needs and available resources. Facilitates patient-care conferences as needed. Implements and coordinates specific caseload strategic interventions to move the patient through the continuum of care, increase medical self-management, and reduce hospital admissions and ER visits.
Participates in departmental orientation, on the job training, and quality improvement programs/initiatives. Provides direct nursing care. Documents care appropriately in the electronic health record. Routinely assess client's status and progress. If progress is static or regressive, determines reason and proactively encourage appropriate adjustments in the care plan and/or services to promote better outcomes in collaboration with the provider. Promotes an environment that is sensitive to cultural diversity. Provide requested reports in a timely fashion that include but are not limited to summaries of services provided and barriers experienced.

Basic Requirements
Education
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.
Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination(external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience
The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:
GS-10: Performing comprehensive clinical assessments in the adult population and chronic disease management; providing HIV Care Management; serving as a consistent member of a multidisciplinary team whose purpose is to conduct care coordination for patients beyond what is offered in the immediate facility (i.e. home health care, hospice care, rehabilitation therapy, skilled nursing facilities, specialty services, durable medical equipment (DMEs), tribal resources, etc.)
and/or
Successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. (You must provide a copy of transcripts with your resume)
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
