The Outpatient Hybrid Clinical Pharmacist is assigned to the Fredericksburg Health Care Center (FxHCC) Pharmacy Section and is responsible for providing appropriate pharmacologic care to Veterans. The duties and responsibilities are carried out throughout all clinical and other Veteran care areas involved with the FxHCC Pharmacy Section.
Duties
DUTIES:
The incumbent in this position represents a clinical pharmacist in this large and complex HCC pharmacy, encompassing outpatient pharmacy, same-day surgery, chemotherapy and infusion services, and numerous subspecialities.
The clinical pharmacist may be assigned on a rotating basis to any of the various sections of the department to meet the needs of the service. This may require rotational shifts. Assignment to these various sections and rotational shifts requires a specialized pharmacy background to perform adequately in the various sections.
The Pharmacy Department operates on weekdays only from 630am-6pm.
The clinical pharmacist is required to be able to demonstrate a knowledge of current pharmacy laws, regulations and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Must be able to interpret VA, Federal and State regulations, Joint Commission, DEA, FDA, Medical Center and Pharmacy department policies and be able to apply the same to given situations.
The incumbent is required to sustain extended periods of stretching, standing and reaching in the course of his/her employment. This occasionally involves lifting moderately heavy items (30-45 lbs.). The incumbent must possess average agility and dexterity, and must be able to work adjusted schedules and rotating shifts as assigned.
The incumbent is required to maintain current knowledge and demonstrate professional pharmacy practices. The incumbent is required to complete all education and training requirements by the VA, VISN and Medical Center within the due date or as required by the Associate Chief of Pharmacy.
CLINICAL:
Assistance in the initial and continuing evaluation of the patient's medication profile and history.
Promotion of the most effective use of medications and works with the practitioners to decrease medical center costs through reduction of non-formulary medications and assure that medications are written according to approved guidelines/protocols.
Obtaining patient medication histories and counseling patients and/or relatives on medications.
Recommendation of appropriate drug therapy including drug, dosage, frequency of administration, duration of therapy, monitoring and cost effectiveness.
Outpatient Area:
Performs all duties normally associated with the pharmacist's professional role in the outpatient environment dealing with the interpretation and clarification of the physician's prescriptions. Must be able to apply VA, state and federal regulations, The Joint Commission, DEA, FDA, Medical Center and Pharmacy department policies as they apply to the prescription order
In the actual interpreting and filling of prescriptions, the pharmacist is expected to follow pharmacy policy as it pertains to dispensed quantities, prior authorization drugs, restricted drugs and appropriate auxiliary labels and patient medication information (PMI) sheets.
Clinically, the pharmacist reviews patient profiles and medical records to determine appropriate usages, dosages, and collects statistical clinical data for drug audits. The pharmacist reviews the patient profile for drug-drug interactions, drug-lab interactions and drug-food interactions. The pharmacist also reviews the patient profile for therapeutic duplications and potential adverse drug actions. The pharmacist much interview patients to obtain a drug history (VA or non-VA dispensed drugs), emphasize compliance, schedule drug administration and correct usage.
Hybrid area:
IV Admixture for chemotherapy and infusion center patients, including rheumatology, renal, GI, and other specialties.
Omnicell stocking and PAR level monitoring;
Evaluation of patients in a variety of subspecialties including same-day surgery, PACT, Mental Health, Spinal Cord Injury, Home Based Primary Care
Work Schedule: Monday- Friday only. Tours will vary but will be between the hours of 630am -6pm

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such licens e by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Experience or Education.In addition to the basic requirements, candidates must meet one of the following:
1. 1 year of experience equivalent to the next lower grade level, or
2. Completion of an ACPE-accredited Pharm.D. program.
Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Experience in outpatient pharmacy is preferred. Prior VA experience in Pharmacy preferred.
