The incumbent serves as the Program Coordinator at the GS-12 grade and is administratively responsible for the Homeless Patient Aligned Care Team (HPACT).The incumbent develops policies and procedures for program operation and prepares reports and statistics for the facility, VISN, and national use. The incumbent manages and controls the use of data related to the day to day functioning and program evaluation.

Coordinates the team staff meetings and the care huddles; oversees the HPACT consult process, and oversees the clinic schedule. Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track program activities including visits, referrals, admission/discharges, inpatient days, patient demographics, and patient satisfaction. The incumbent evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and processes. The incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Performs specialized treatment of patients with complex mental and physical illnesses and homelessness. Incorporates complex multiple causation in differential diagnosis and treatment of veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Teaches and mentors staff and students in the special are of practice and may provide supervision for licensure or specialty certifications. Provides both informal and formal education to HPACT staff on any changes in VA policy. Collaborates with members of the HCHV team, the GPD programs, the HPACT team, and other professional specialty clinics to provide comprehensive care to Veterans. Performs screening/assessments, treatment planning/goal setting, referrals to service providers, advocacy, community planning, education/health promotion and prevention and supportive counseling for Veterans enrolled in the HPACT. Conducts individual and family clinical interventions to facilitate the identification of substance abuse, psychiatric illness, and social needs as treatment issues. Responsible for documenting patient and family contacts in the Veteran's medical record via CPRS. Participates in program planning, development and evaluation. Responsible for advising the section supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes. Collaborate with VA staff and community agencies to strive to develop more effective and efficient ways of delivering healthcare services to homeless Veterans. Performs other duties as assigned. Work Schedule: Monday through Friday: 8:00am to 4:30pm Recruitment Incentive (Sign-on Bonus): May be authorized. Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: . Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).6 May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met for this position. Social Worker(Program Coordinator),GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: ???????The work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices, training rooms, and libraries. The work area is adequately lighted, heated, and ventilated. The incumbent must also work outdoors in all types of weather. The work is primarily sedentary, although some physical effort may be required, e.g., walking, standing, climbing ladders, stooping, kneeling, and carrying light items such as manuals or briefcases, or driving or traveling by motor vehicle.
