This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Duties: The BHIP Social Work Team Lead/Case Manager is a professional social worker assigned to a BHIP team at the VA SORCC main campus or a Community Based Outpatient Clinic (CBOC) in Grants Pass or Klamath Falls, Oregon. The incumbent is responsible for providing thorough DSM-5 diagnostic and biopsychosocial assessment, case management, walk-in/same-day services, group psychotherapy, and other clinical tasks as needed for a caseload of up to 1000 Veterans receiving services within BHIP. Case management includes tracking and monitoring all activities to provide appropriate treatment planning. Additionally, the BHIP Team Lead facilitates weekly huddles, case conferences, and other administrative tasks related to the smooth operation of the BHIP team. The incumbent coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service. Serves as the Mental Health Treatment Coordinator (MHTC) for assigned Veterans and conducts the duties of this role. Demonstrates sensitivity to diversity and possesses multicultural counseling skills. The incumbent is responsible for following local policy and standard operating procedures for the BHSL. This includes following policies and documentation requirements as required for CARF and The Joint Commission accreditation. The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for making recommendations on program procedures and/or policy changes based upon work experiences. The incumbent takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. The Social Worker will conduct training for staff, students and community providers regarding Primary Care issues, will provide education, information and liaison with resources for Veterans in the community, and will provide technical assistance and educational sessions to community organizations as needed. Other duties and responsibilities may be assigned as needed for the benefit of the Veteran, family, and quality of services provided. The individual works full-time in an outpatient clinic setting but may be required to work an irregular tour of duty, weekends, depending on the needs of the Clinic and service line. The incumbent is responsible for completing the following duties/responsibilities: a. Triage incoming BHIP referrals and self-assign MHTC for up to 1000 Veterans per BHIP team. b. Engage in information gathering, problem identification, DSM-5 diagnostic assessment, biopsychosocial assessment, and using measurement based care. Fully utilizes the current DSM-5 in making diagnoses and formulation of treatment goals and application of appropriate clinical intervention using professional counseling practices. Triages Veterans amongst BHIP team members for ongoing care (psychiatric medication management, psychotherapy, etc.). c. Provide individual and group counseling, wellness/prevention education, and facilitate patient/family support groups as appropriate. Utilizes a wide variety of evidence-based, time-limited interventions. Treatment approaches include but are not limited to cognitive-behavioral therapy, acceptance and commitment therapy, dialectical behavioral therapy, and interpersonal psychotherapy. d. Participate in the treatment planning process with other disciplines. Makes professional evaluations, decisions, and recommendations for treatment planning and implementation. Establishes goals/treatment through a collaborative process with the patient utilizing advanced counseling skills, including evidenced-based practices. e. Provide case management to Veterans and families, as needed, throughout the continuum of care. Provides complex crisis intervention and stabilization to patients who are in psychological distress. f. Document social work interventions and activities in patient's clinical record and ensure appropriate hand-off when transferring of patient care. Documentation is of the highest quality, representative of BHIP and clinical social work standards of care. g. other duties as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/ Grade Determinations: Senior Social Worker, GS-12 Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
