The Oklahoma City VA Health Care System is recruiting for a Clinical Pharmacist position at the Oklahoma City VA Medical Center. Clinical Pharmacists are generally assigned to Inpatient, Outpatient, or Procurement areas to perform clinical functions as assigned by the supervisor.

Order verification includes assessment of computer-generated warnings, flags, pop-ups and ensuring prescriptions are safe in all aspects for patient administration. Provide dose recommendations, enter new orders, refills, partials, traveling veteran refills, enter/request renewals, process therapeutic substitutions, provide supplies, OTCs or enter non-Formulary medications into the pharmacy computer package per policy. May take medication histories, perform medication reconciliation, enter/document patient information, review data, or similar in the electronic medical record. This includes completing notes, consults, or similar about patient care issues such as VA or non-VA drugs, ADEs, reported suicide, REMS, mail returns, CMOP cancelled orders, CS losses or other required communication related to pharmacy responsibilities. Provide expert drug related information to patient counseling, product demonstration, and problem solve patient concerns related to medications or supplies provided by pharmacy. They should be well versed in oral and written communication and be able to communicate using appropriate population specific competencies to patients and professional communication to other VA staff. Provide expert drug related information to VA staff on timing, duration, and dosing of medications, clarifying orders, problem solving prescription issues, disseminating Formulary or non-Formulation criteria for use, providing information on drug shortages, conversions, controlled substance disposition, or other pharmacy processes like PADRs, order sets, quick orders or similar. Work in all aspects of the drug supply chain process including procurement, (distribution center orders, contract orders, purchase card orders or similar for CS, non-CS medications, supplies, equipment, CSPs, office supplies, or any need of pharmacy), resolving partial orders, amending POs, cancelling orders, crediting orders, receiving product, returning product, inventory assessment/management, re-stocking, billing, budget, purchase card resolution, fiscal audits, loan/borrow procedures, SVC procedures, supporting emergency preparedness areas such as the all hazard cache, or any other as assigned by the supervisor. Coordinate Pharmacy Technicians workload. Expected to understand drug automation equipment including preventative maintenance and general problem-solving activities. They are expected to monitor tech workstations, use automation filling/verification workstations and window workstations. They should understand the preparation or dispensing process related to automated dispensing cabinets (ADCs), carts, trays, tablet counters, IV hoods, Isolators, label printers and any other type of pharmacy equipment. Able to retrieve information for medical decision making, use on-line applications (for example safety data sheets), have basic computer skills to be proficient with software applications including but not limited to pharmacy verification apps, procurement apps, billing apps, inventory apps. phone systems, or other commonly used pharmacy equipment like unit dose machines, pill packagers, pill counters, re-packaging equipment, phone systems, or the like. Basic Requirements Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the 1 graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed. Grade Determination. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. GS-11 Pharmacist Experience, Education, and Licensure. None beyond the basic requirements GS-12 Clinical Pharmacist (Full Performance Level) Experience or Education. In addition to the basic requirements, candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. -AND- In addition to meeting the experience for the GS-12 level, Pharmacists assigned to this position must demonstrate the following Knowledge, Skills and Abilities: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-11 to GS-12. Physical Requirements: The position requires adherence to all policies and procedures of the department, medical center, and VHA. The position requires walking, standing, and sitting for long periods of time. The position also requires climbing stairs, shelf stocking, lifting up to 50 pounds, transporting pharmacy items, dexterity of the hand and wrist, and arm strength to perform all pharmacy related activities. All activities are conducted with the goal of caring for the Veteran and the incumbent will remain within guidelines of practice, personal safety and security as established by the Pharmacy Department.
