This position functions in the Pharmacy Service Clinical Section specifically related to Administration duties. The clinical pharmacy technician is a certified pharmacy technician with advanced skills who will provide administrative, clerical, and clinical support to the administration office staff and Clinical Pharmacist Practitioner (CPP). Responsibilities will also include full range of technical support functions related to Pharmacy clinics.

All assigned clinical activities and function shall be completed in accordance with approved clinical pharmacy protocols or procedures: Assigned activities include but not limited to: Medication History: Pull data via the electronic health record (EHR) on all new patients who are new to the Patient Aligned Care Team (PACT) and add to excel sheet to complete medication history. Interviews patients (families/caregivers as appropriate), to develop an accurate list of medications, strengths, and dosages the patient has been taking. Interviews patients for any over the counter medications and/or herbal product. Review Joint Legacy Viewer (JLV) to see if patient has been receiving medications from another VA. Document adverse reactions and allergies that the patient reports. Review any patient compliance issues based on the interview and refill history. Document the compiled list in the electronic health record. Clinical: Routine review of laboratory reports for selected patient groups and identification of patients with lab values beyond normal limits or within certain parameters as specified by the CPP. The clinical pharmacy technician will identify patients to the assigned CPP for evaluation. Routine review of the computer medication profile for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies/adverse drug reactions. Report identified problem patients to the assigned CPP for evaluation. Routine review of the patient medical chart and medication profile to identify poly- pharmacy problems. Report identified problems to the assigned CPP for evaluation. Perform routine telephone communication with pharmacy clinic patients in conjunction with Computerized Patient Record System (CPRS) and medication profile review to obtain pertinent information requested by the CPP. Review patient dashboards as assigned to identify potential patients and report to the Clinical Pharmacy Supervisor. Administrative Activities: Routinely review scheduling/appointment reports and computerized medical records to ensure appropriate consult management and patient follow-up. Coordinate administrative/clerical support for pharmacy clinics with Medical Support Assistants (MSAs) and other departments as assigned. Participate in quality assurance, internal audits, and performance improvement processes and programs as assigned and in a timely manner. Assist in data collection for drug utilization reviews as assigned in a timely manner. Input new, refill, and mail prescriptions into the computer system after the prescriptions have been reviewed for appropriateness by a pharmacist. Prescriptions entered by the clinical pharmacy technician shall be signed off by a CPP with prescribing privileges prior to final processing and verification. Assist CPPs with LEAF requests for clinic closures and/or modifications as needed. Program Management: Serves on medical center and/or Veterans Integrated Service Networks (VISN) committees as requested. Adheres to all medical center and department policies and regulations. Performs all the duties of a pharmacy technician at the lower grades as assigned. Assists in other patient care areas as deemed necessary to maintain the consistency of overall pharmacy services. Other pharmacy related duties as needed to help fulfill our mission and abide by our organization's values. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Be a citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with 38 U.S.C. § 7407(a). Education or Experience: None required. For all grade levels and positions that accept both education and experience to qualify, equivalent combinations of qualifying education and experience that total at least 100% are qualifying. The combined percentage is determined by adding total qualifying experience as a percentage of the experience required for the grade level to the education as a percentage of the education required for the grade level. Certification: For positions above the full performance level, the employee must pass a national certification exam and hold an active national certification through either: Pharmacy Technician Certification Board (PTCB), Certified Pharmacy Technician (CPhT). National Healthcareer Association (NHA), Certified Pharmacy Technician (ExCPT). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grandfathering Provision: Pharmacy technicians employed in VHA on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements in this standard but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Employees may be reassigned, promoted up to and including the full performance (journey) level or changed to lower grade within the occupation but may not be promoted beyond the journeyman level or placed in supervisory or managerial positions. Employees in an occupation that requires a licensure/certification/ registration only at higher grade levels must meet the licensure/ certification/registration requirement before they can be promoted to those higher grade levels. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed on a temporary or permanent basis until they fully meet the basic requirements of the standard. Employees who are converted to title 38 hybrid status under this provision and subsequently leave the occupation lose protected status and must meet the full VA qualification standard requirements in effect at the time of re-entry to the occupation. Employees initially grandfathered into this occupation who subsequently obtain additional education and/or licensure/certification/registration that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. Grade Determinations: Pharmacy Technician, GS-08 Experience. One year of progressively complex experience equivalent to the lower grade level (GS-07). Certification. For positions above the full performance level, the employee must pass a national certification exam and hold an active national certification through either: PTCB as a CPhT. NHA as a ExCPT. Knowledge, Skills and Abilities (KSAs). In addition to the experience above, the candidate must demonstrate the following KSAs and meet any additional KSAs for the assignment, if indicated: Comprehensive knowledge of concepts, principles, methodology and policies in a specialized area or section of pharmacy (such as sterile compounding, controlled substances, automation, clinical pharmacy, contact center, etc.) Note: See Specialized Area.in the Definitions Section. Skill in training and orienting new and existing pharmacy facility employees and students on proper policies and procedures. Ability to evaluate, analyze and coordinate workflow and work activities within a specialized area or section of the pharmacy. Preferred Experience: Vast knowledge of pharmacy-specific community care processes. Customer service and communication skills are a must. Physical Requirements: Physical requirements for this position are as follows: Light lifting, 15-25 pounds with occasional heavier lifting. Light carrying, 15-25 pounds with occasional heavier carrying. Straight pulling (up to 2 hours). Pushing (up to 2 hours). Reaching above shoulder. Use of fingers. Repeated bending.
