This position is located at Claremore Indian Hospital (CIH) in Claremore OK. The incumbent functions under the directive of the Chief Nurse Executive (CNE) and assists with planning, organizing, directing and monitoring the activities of the Cardiology Imaging Clinic.
This position is being announced concurrently under Direct Hire Authority procedures vacancy announcement number IHS-24-OK-12527111-ESEP/MP; for eligibility requirements.
Duties
Collaborate with cardiologist, primary care providers, clinic nurses, public health nursing, outside hospital and/or clinic entities, pharmacist, radiology, laboratory, and social services to ensure that the patient's care needs are completed in a timely and cost effective manner to maximize each visit.
Coordinates clinic visits for referred patients with other members of the multidisciplinary team in order to optimize care.
Conducts pre-visit planning for scheduled patients and communicates the patient needs and recommended treatment to care team members.
Serves as primary contact/clinical liaison between referral hospitals and/or clinics, treatment facilities and patients and their families to ensure delivery of effective and appropriate treatment in accordance with established cardiology plan of care.
Provides an advanced level of specialized nursing care patients in the Cardiology Clinic Service. Assesses the patient information including physical and psychological needs, by evaluating patient records and history.
Provides data for Quality Assurance Program. Participates in department process improvement activities and reports as required. Participates in developing policies and procedures. Performs data collection, compiles data, creates, accesses, updates, and maintains security of computer databases to facilitate patient outcomes. Generates statistical reports regarding access to and outcomes of care provided for the purposes of process improvement and to meet the requirements of federal accrediting agencies.

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Must submit copy of transcript. Applications will be considered incomplete without proof. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. AND
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:
GS-10: 1 year of specialized experience documenting cardiovascular patient education and utilizing education codes in an electronic health record, including patient education discussions as a chart review as well as ensuring patient/families have all necessary information for their related diagnosis and cardiovascular procedure(s). Case management of patients with chronic health conditions. Alerting physicians of potential problems; plan and coordinate nursing care accordingly, which requires knowledge, interpretation and performance of cardiology tests (EKG, stress tests, echocardiogram, Computed Tomography Angiography (CTA), laboratory values), cardiac monitoring, 12 lead EKGs and interpretation. Administration of cardiac medications utilized during a cardiology test or prescribed by the attending cardiologist. Recognizing critical situation and initiating/performing Advance Cardiopulmonary Life Support (ACLS) per the American Heart Association's guidelines.
