The Geriatrics and Extended Care (GEC) Social Work Program Coordinator will have primary administrative and clinical responsibility for assigned GEC programs, which may include: ADHC (Adult Day Health Care), PACE (Programs of All-Inclusive Care for the Elderly), Homemaker/Home Health Aide (H/HHA), In-patient respite and Contract Nursing Home (CNH).

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Program Coordination and Development: The Geriatric Extended Care (GEC ) Social Work Coordinator will provide the following program oversight functions: Serves as point of contact for all inquires regrading the ADHC (Adult Day Health Care), PACE (Programs of All Inclusive Care for the Elderly), In-patient respite and Homemaker/Home Health Aide (H/HHA) programs. This may include stakeholders (e.g. veterans, families, community hospitals, nursing homes, and community agencies) and Congressional inquires. Screens and assesses Veteran and his/her caregivers psychosocial needs and barriers in order to facilitate access to appropriate VA and community resources. Ensure these programs are administratively and clinically in compliance with all National and local VA policy and directives responsible for assuring that the placement of Veterans into GEC programs meets VA administrative and clinical eligibility Treatment Planning/Goal Setting From screening/assessment information, the GEC Coordinator determines realistic goals with the Veteran and his/her caregiver, including such factors as a) the appropriate type of intervention and/or level of care, b) the setting conducive to providing such intervention or care, c) the need for intervention to allow for the least restrictive level of care, d) transportation options/barriers and e) the steps to obtaining resources (VA or community) that make the achievement of goals possible. Case Management- The GEC Coordinator will provide case management services as needed to Veterans and their caregivers. As a lead coordinator for the CCICM program, the GEC Coordinator may be assigned as a lead coordinator for Veterans outside of the GEC services. Resource Usage/Development As a subject matter expert on the services and resources available to Veterans through both VA and the community, the GEC Coordinator will monitor patterns of resource availability, identifying unmet needs and the resultant impact on the Veteran's pyschosocial functioning GEC Coordinator will communicate with appropriate stakeholders to encourage the development of new and innovative resources. Crisis Intervention The GEC Coordinator will assess and intervene in any medical or psychiatric crises presented by the Veteran and participate in developing crisis management and safety plans to minimize the event of future crises. Education/Health promotion and prevention The GEC Coordinator will provide education about VA and community resources, entitlement programs, VA benefits and Advance Directives/Living, completing the advanced directive or living will as requested. The GEC Coordinator will refer Veterans and their families/caregivers to the appropriate interdisciplinary team member for identified health education needs. Non-patient care responsibilities: The social worker will enter all Veteran/family contacts in the electronic record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to insure information on the patient is shared with other VA staff. This information will be reviewed by a supervisor or peer reviewer on a regular basis. The social worker will establish and maintain positive working relationships with employees, volunteers, patients/consumers and other stakeholders within the VA and in outside community agencies. The social worker will attend all appropriate staff meetings (interdisciplinary treatment team meetings, social work staff meetings, other committees as appropriate or as assigned) and perform other duties as required to meet the mission of the program or the VA in general. The social worker will collaborate in performance improvement processes and comply with performance measures as required by the VA for their specific population and/or program. The social worker will participate in activities that solicit feedback from patients and work to enhance services as needed. Work Schedule: Monday- Friday, 8am-4:30pm Telework: Regular Telework 1 day a month, Telework will be discussed following successful completion of orientation and training. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies such as papers, books, or small parts. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others. This position also requires close visual acuity (near vision correctable at 13" to 16") to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, hearing (aid permitted), and clear distinctive speech are all required.
