The HUD-VASH program at the Cheyenne VA Health Care System is recruiting for one Senior Social Worker to join their team. The incumbent provides specialized services to a high-complexity caseload of Veterans, as well as functions as a subject matter expert as the Lead HUD-VASH Social Worker for their assigned team. This position is located at the Cheyenne VA and requires that the employee travels regularly, requiring operation of a government vehicle.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU List of Major Duties: Functions as an advanced practice Senior Social Worker providing specialized interventions and treatment modalities to homeless Veterans and their families and Veterans at risk for homelessness. Performs one or more of the following: consultation to team members regarding complex multiple causation in differential diagnosis and treatment, process improvement tasks, supervision of Social Workers for advanced practice licensure and/or the graduate trainee program, act as a mentor for less experienced Social Workers, participate or chair committees or groups that impact social work practice and/or lead roll-out of evidence-based practices. The incumbent will model the advanced level of practice and provide consultation to less experienced Social Workers and other disciplines. Works closely with the VAHCS's Homeless Programs, residential and community care programs, and other community partners that provide services to Veterans experiencing homelessness to ensure that coordination between these entities is facilitated and Veteran direct care, referral, and follow up is contiguous and based on a continuum of services. Provides clinical psychosocial and case management services that are at a professional, independent practice level for eligible Veterans and their family members in the program. Functions as a Lead HUD-VASH Senior Social Worker and subject matter expert for the HUD-VASH program. Provides programmatic support and consultation for HUD-VASH Social Workers and other disciplines. May provide trainings to staff related to coding, CPRS documentation and best practices within the field. May act as surrogate to the HUD-VASH Program Manager, for CPRS purposes, cosigning notes/consults/orders and providing clinical guidance when appropriate or necessary. Attends national HUD-VASH Program conference calls and attends training offered by the HUD-VASH and HCHV programs. Provides services to Veterans with complex mental health, medical, co-morbid disorders or who display increased risk for mild to moderate disruptive behavior or who may be at an elevated risk for suicidal thoughts or behavior Follows national program policy, local facility policy, and Mental Health Service Line SOPs as it relates to services provided, CPRS and HOMES documentation, and program/facility requirements. This includes following policies and documentation requirements as required for CARF and JCAHO accreditation. Assists in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. Is required to maintain current BLS and/or First Aid certification and remain current on all licensures. Performs other related duties as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determination: GS 12 Senior Social Worker: Experience: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Several years of working experience in the HUD-VASH program, as well as the ability to be a HUD-VASH program subject matter expert to assist with case consultations and other program assistance needs. Physical Requirements: Typically, the employee sits comfortably indoors to do the work. This position requires potentially long periods of continued walking; standing; sitting, bending; carrying of light items such as papers, books, small parts; or driving an automobile; etc. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16", hearing (aid permitted), clear distinctive speech.
