The VA Black Hills Health Care System is recruiting for 1 full-time Social Worker (Program Coordinator) for Care Coordination and Integrated Case Management under the Social Work Service Line. This position may be located at either the Fort Meade VA Medical Center or the Hot Springs VA Medical Center.
Duties
The Program Coordinator assigned to the CC/ICM model of care is responsible for building networks, case identification through evaluation and data analysis, direct case management services, facilitation of the Care Coordination Review Committee (CCRT), and direct collaboration with nursing and social work CC/ICM leads. The health care and psychosocial needs of the population served are complex and require a high degree of independence, clinical oversight, and creative problem solving. The CC/ICM SW Coordinator will need to build and foster networks for referral and care coordination, provide support and education to staff throughout the medical center, and work collaborative within multiple care settings to meet the needs of those served.
This position is aligned under the Social Work Executive and Associate Director of Patient Care Services. The CC/ICM SW Coordinator will interface with all Integrated Care Communities (ICC) across the health care system and in turn will be aligned under the Social Work Executive. The duties and responsibilities of this position require and authorize the individual to format, determine, and influence the policies of the program. Fifty percent of his/her time will be sent in administrative duties. Given the responsibilities and duties of the position, s/he is considered a management official.
The incumbent must function with a high degree of independence and knowledge of VHA regulations in managing medically and psycho-socially complex cases. The incumbent is responsible for advance level case management and coordination of care for patients across the continuum of care, including initial assessments, recommendations, VA & community referrals, Hospice/Palliative care, supportive counseling, grief/bereavement for patient's families/friends, care coordination, and discharge planning.
She/he will collaborate and facilitate referrals based upon veterans needs and eligibility.
Independently conducts psychosocial assessments. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team(s) who are involved.
Develops and implements methods for measuring effectiveness of nursing and social work practice.
Communicates effectively, both orally and in writing, with people from varied backgrounds.
Serves as a subject matter expert on case management and the CC/ICM model of care.
Develops and implements policies and procedures specific to the health care system and ensures compliance with local facility, VISN, the Social Work Services/Case Management services, Patient Experience Initiatives, VA Central Office (VACO) directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed.
Ensure full collaboration with nursing and social work colleagues across the health care system.
Identifies Veteran cohorts across the health care system with the highest opportunity and highest need. Including development and implementation of a monitoring process for ongoing screening and identified cohort(s).
Engages in the development of facility wide CC/ICM education planning for key stakeholders.
Ensures compliance with VHA Directive 1110.04, Integrated Case Management Standards of Practice and other associated facility and national policies.
Serves as the primary lead for the coordination and facilitation of the Care Coordination Review Team.
Conducts psycho-social assessments, & leads care coordination efforts through direct practice and by mentoring, modeling, and educating others.
Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
Work Schedule: Monday-Friday, 7:30 a.m. - 4:00 p.m.

Pay: Competitive salary and regular salary increases.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-12 Social Worker (Program Coordinator)
Experience and Education. One year of experience equivalent to the GS-11 grade level, which is defined as: Working knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services; Independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds; Working knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan; Independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals; Providing consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
