The Social Services Representative functions as a Mental Health Rehabilitation Technician in support of the HUD-VASH program within the HCHV (Health Care for Homeless Veterans) program. The position is located within the Mental Health Service, Homeless Programs with the Dayton VA Medical Center (VAMC) located in Dayton, OH.

Duties

Duties: Incumbent's primary job assignment consist of providing outreach and case management services to the homeless Veterans population, as well as working very closely with other VA and community resource providers. Manages and Organizes Information: Incumbent will be responsible for documenting all viable encounters for patient visits, tracking patient data in national Homeless Operations Management Evaluation System (HOMES) and the Veteran's clinical medical record. Works with the HUD-VASH interdisciplinary team to ensure that Veterans have access to services for homelessness and prevention of homelessness and assist with the goal of expediting housing placements as rapidly as possible. Facilitates communication to coordinate with the HUD-VASH Team and HUD-VASH Supervisor to help the team identify and provide other services as it applies to Veterans in need of shelter, medical care, and basic daily living needs. Incumbent assists Veterans in navigating the HUD-VASH process including assisting in obtaining documentation, attending Public Housing Authority (PHA) appointments, transporting Veterans to appointments with the assigned HUD-VASH Coordinator or other VA providers and community partners. Incumbent is expected to be instrumental in transporting Veterans and assisting them in obtaining and maintaining VA services, including transporting Veterans participating in Homeless Programs via government vehicle for housing search appointments, assist with move-ins and obtaining furniture and goods for the Veteran's new housing under the direction of the HUD-VASH Case Manager, HUD-VASH Supervisor and/or Homeless Programs Manager. Incumbent must possess the judgement to interpret forms related to securing housing with the Public Housing Authority (PHA) and/or private landlords. Incumbent must be able to navigate complex systems including the Social Security Administration (SSA), Veteran Service Organizations (VSO), Food Banks, and other community resources. The Incumbent will work closely with the assigned Homeless Programs staff to assist the Veteran with referral and linkage to VA and community resources to treat and/or alleviate these problems (e.g., mental health, SUDS, homelessness). Incumbent supports the Veteran's housing and other co-occurring treatment goals. Incumbent is expected to comply with all VHA Regulations and Directives and Medical Center Policies and accreditation and regulatory bodies, including workload requirements, documentation policies. Incumbent may assist in collecting information for the clinicians to complete HUD-VASH Orientations, HCV Applications, HUD-VASH Voucher Briefings, HUD-VASH Lease Signings and HUD-VASH Annual Recertifications. Incumbent will facilitate communication with Homeless Programs team members on individual Veteran cases regarding the Veteran and will provide appropriate input for development of the Veteran's treatment plan. Incumbent will demonstrate an ability to develop a rapport with the Veterans and their families as well as VA and community providers. Incumbent will perform follow up services in the community and the VA Medical Center with connecting Veterans for resources. Incumbent will work closely with the Homeless Programs Teams in addressing the complex needs of this Veteran population. Incumbent accepts direction, guidance, and clinical supervision from the HUD-VASH Supervisor, and seeks consultation, guidance and recommendations regarding Veteran treatment and care from relevant Homeless Programs staff. Incumbent may conduct or co-facilitate in-person or virtual group sessions and/or basic recovery services with Veterans based on Veteran's independent living skills, housing, and financial assistance resources. The incumbent will be required to perform most (75%) of their job duties in the community transporting Veteran participants and will be required to utilize a GSA vehicle while performing such duties Work Schedule: Monday-Friday, 8:00 am-4:30 pm Telework: Ad-hoc Virtual: This is not a virtual position. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/16/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: One year of specialized experience equivalent to at least the next lower level in the normal line of progression is qualifying for positions above the GS-05 level. Specialized experience is experience in a program of direct social welfare services that required the application of program policy and operations knowledge in providing social welfare services and/or assisting social workers.. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have Graduate Education: Major study -- social sciences. One full academic year of graduate study is qualifying for GS-07. Graduate education is not creditable for positions above the GS-07 level. OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond Graduate Level. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities.
