This position is located within the Mental Health Service, Homeless Programs with the Dayton VA Medical Center (VAMC) located in Dayton, OH.

Duties

***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** Duties include but may not be limited to: Provides supportive outreach services to Veterans with challenging needs who are homeless, at imminent risk of homelessness or being provided case management services. Provide direct, integrated, immediate services to Veterans in need. Instrumental in transporting Veterans and assisting them in obtaining and maintaining VA services. Possess the judgement to interpret forms related to securing housing with the Public Housing Authority (PHA) and/or private landlords. Perform follow up services in the community and the VA Medical Center with connecting Veterans for resources. Work Schedule: Full Time; Monday-Friday, 8:00am - 4:30pm

QUALIFICATIONS To apply for this position, you must have the following qualifications: English Language Proficiency - In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position. AND EDUCATIONAL REQUIREMENT: Do you possess a Bachelor's degree from an accredited college or university in major fields of study such as: social welfare, sociology, psychology, public administration, philosophy, education, anthropology, history, economics and/or English? OR SPECIALIZED EXPERIENCE: One year of specialized experience equivalent to at least the next lower grade level (GS-6) in the normal line of progression of the position and Two years of experience as a social welfare worker or social work assistant in a voluntary, public, social, or health agency, which must have included 1 year in a medical setting. Specialized experience is experience in a program of direct social welfare services that required the application of program policy and operations knowledge in providing social welfare services and/or assisting social workers. NOTE: You must submit a copy of your transcript with your application. Education cannot be credited without documentation. (Resume(s) must clearly support the duties and experience required for this position. We will not make assumptions regarding experience.) AND SPECIALIZED EXPERIENCE: At least one (1) year of specialized experience equivalent to the next lower grade level GS 6) in Federal Service that includes performing the following duties: Utilize general counseling techniques and crisis intervention to use in support of MSW social workers' clinical work.; Gather (social history) to identify needs of the problems/issues confronting the patient and recommendations regarding strength-based approach to resolution.; Document activities in the patient's medical record in accordance with policies; assisting with applications/paperwork, forms (including but not limited to Advance Directive paperwork).; Gather information, handling details around case management/care coordination including patient benefits, Medicaid applications, assisted living facilities and contract nursing home placements. Assist with following up on the acquisition of documents, reports, or information needed for social work treatment. You will be rated on the following Competencies as part of the assessment questionnaire for this position: Computer SkillsCustomer ServiceManages and Organizes Information IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Physical Requirements: The work is active and is performed in a clinical setting as well as in the community. The incumbent serves a Veteran population with complex needs. Normal safety precautions and practices are required to working in a healthcare environment. This position requires emotional ability to work effectively with multi-tasking and high volume of patient flow. Other demands include, reaching, bending, stretching, walking, and carrying light loads of supplies. This position requires significant travel in a government vehicle and transporting Veterans in a government vehicle. The incumbent will be required to perform most (75%) of their job duties in the community and will be required to utilize a GSA vehicle while performing such duties. The Social Services Representative Mental Health Rehabilitation Technician will successfully complete all facility requirements to drive a GSA Vehicle and transport Veterans. This includes all required physicals, including a pre-employment physical. In addition, the incumbent will successfully complete all facility required TMS and other trainings to drive a GSA Vehicle and transport Veterans.
