The Connected Care Education and Implementation Specialist is a national leader in providing innovative strategies, guidance, implementation of educational programming, and related functions. This position is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines influence organizational mission and health care for the national Office of Connected Care Program.

Duties

Duties include, but may not be limited to: Develop and implement a VHA-wide education portfolio for Connected Care/Telehealth staff and stakeholders and related functions with particular emphasis on role-based core competency curricula through multi-level complexity of Telehealth training, implementation, and outreach products and services that meet VA, OCC internal Conditions of Participation as well as the Joint Accreditation for Inter-professional Education and other related Accreditation standards. Provide performance support for multidisciplinary groups that include but are not limited to VISN Telehealth Leads/Program Managers, physicians, physician assistants, nurse practitioners, registered nurses, lead care coordinators, care coordinators, facility telehealth coordinators, program support staff, and telehealth clinical technicians. Facilitate, coordinate, and promote relationships within all levels of VA, professional organizations, certifying bodies, and program directors, as well as foster new relationships with internal and external programs and practice communities through the performance of outreach strategies such as leading Forums, Office Hours, demonstration sessions, and other related activities to support the delivery of high-quality care. Implementation is the method to promote the systematic uptake and outreach of evidence-based practices into routine practice and competent staff, to improve the quality and effectiveness of Veteran Care and expand virtual care. Ensure national implementation and continuous management to assure integration of concepts and clinical standards related to connected care/telehealth, systems redesign, evidence-based practice, and transparency in providing these services safely to Veterans. Develop and implement strategies to effectively implement educational resources associated with the national Connected Care/Telehealth Program. Work Schedule: Monday-Friday, 8:00am-4:30pm EST Remote/Virtual: This position is not eligible for Remote work. Telework: This position is eligible for Telework. Travel: Occasional. Relocation/Recruitment Incentives: Not authorized. Permanent Change of Station (PCS): Not authorized.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience and Certifications - -4-5 years of direct telehealth-related experience; national level participation in telehealth committees, work-groups, presentations; significant instruction, coaching, mentoring, and training experience with staff to maximize utilization of healthcare resources through the effective application of knowledge and skill in telehealth, virtual care technologies, health informatics evidence-based care, system redesign, and case management; national level expertise in telehealth and connected care technologies; expertise in course design, development in Telehealth modalities with focus on synchronous video. Experience with competency-based training and education courses, modules, resources, tools, products, and services. Experience providing performance support for multidisciplinary groups that include but are not limited to Telehealth Leads/Program Managers, physicians, physician assistants, nurse practitioners, registered nurses, lead care coordinators, care coordinators, facility telehealth coordinators, program support staff, and/or telehealth clinical technicians. Exceptional verbal and written skills to facilitate, coordinate, and promote relationships within all levels of VA, professional organizations, certifying bodies, and program directors, as well as foster new relationships with internal and external programs and practice communities through the performance of outreach strategies such as leading Forums, Office Hours, demonstration sessions, and other related activities to support the delivery of high-quality care. Experience independently collaborating programs and/or services across multiple sites and across a continuum of care, taking a collaborative and leadership role in working with all levels of staff in developing and implementing an education portfolio. Experience with data mining, preparation and presentation of reports for multiple levels of the organization including facility, VISN and national audiences. Proficient with use of VSSC, CDW, Power BI. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 year's of nursing practice/experience; OR a Doctoral degree and approximately 3-4 year's of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. In the performance of duties, the incumbent may be required to drive and/or ride in GSA-vehicles.
