The incumbent has responsibility for coordinating and directing the HBPC Program, including providing clinical supervision for interdisciplinary staffing, designating work assignments, delineating, and maintaining HBPC budget, overseeing treatment services provided, and implementing policies and procedures that promote quality of care.

Responsible for the day-to-day operation of the HBPC program. Includes planning, directing, budgeting, monitoring, and evaluating the program. Determines HBPC program goals, works independently and with HBPC Medical Director, HBPC Nurse Manager, and Geriatrics & Extended Care (GEC) management in the development of plans for the attainment of these goals.
Identifies program resource requirements, ensures appropriate acquisition and allocation of essential resources and account for the effective utilization of available resources in support of program activities.
Identifies and implements changes in the organizational structure of the HBPC program essential to ensuring confluence with changes in the treatment needs of the patient population.
Considers from an informed perspective, a broad spectrum of factors when making programmatic decisions or recommendations to GEC management including labor-management relations, public policy positions, effects on other VA and non-VA organizations/activities, economic impact, and other similar such considerations.
Ensures effective integration of program efforts/activities with other medical center operations and with community based agencies and activities.
Establishes, comprehends, interprets, and effectively communicates program emphasis, operating policy, and overall treatment philosophy to other operational entities.
Effectively manages general personnel policies and established practices regarding program operations as they relate to key employees and management actions with possible serious repercussions.
Ensures compliance with local and system-wide program policy manuals and directives.
Serves as resource person in the medical center for questions regarding Home Based Primary Care.
Demonstrates leadership in addressing ethical issues that impact patients and staff in or beyond the organization and in the local health care community.
Work Schedule: Monday to Friday 8:00am to 4:30pm

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Qualifications
Basic Requirements:
Citizenship - You must be a citizen of the United States
English Language Proficiency - You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f)
Education and/or Experience Combination -
You must possess a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work.
License or Certification - You must be licensed or certified by a state to independently practice social work at the master's degree level. NOTE: There is no licensure exception at this grade level.
Grandfather Provision. A Social Worker employed in VHA on the effective date of the qualification standard (9/10/2019) who did not meet all the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed, the following provisions apply:
1. Employees may be reassigned, promoted up to and including the full performance (journey) level or changed to lower grade within the occupation but may not be promoted beyond the journeyman level or placed in supervisory or managerial positions.
2. Employees in an occupation that requires a licensure/certification/ registration only at higher grade levels must meet the licensure/ certification/registration requirement before they can be promoted to those higher grade levels.
3. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed on a temporary or permanent basis until they fully meet the basic requirements of the standard.
4. Employees who are converted to title 38 hybrid status under this provision and subsequently leave the occupation lose protected status and must meet the full VA qualification standard requirements in effect at the time of re-entry to the occupation.
5. Employees initially grandfathered into this occupation who subsequently obtain additional education and/or licensure/certification/registration that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
Social Worker (Program Coordinator) GS-0185-13
Licensure Requirement: You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Experience Requirement: You must possess one year of experience equivalent to the GS-12 grade level. Examples of specialized experience include, but are not limited to: Serving as the sole practitioner in a specialty program and typically provide direct patient care services in the program area; oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics; may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery; provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.
In addition to the experience above, you must demonstrate the following Knowledge, Skills and Abilities:
Skill in coordinating and implementing one or more specialty program(s), across the facility.
Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
Knowledge of organizational structure and impact on program operations.
Preferred Experience:
Experience in crisis intervention, case management, resources, community referrals and discharge planning.
Knowledge and experience in program development, performance measures, and budgets.
Knowledge and experience in the role of administration and management, addressing staffing and performance needs, productivity, and disciplinary action.
