The Social Work Program Coordinator functions as the Medical Center's Coordinator/Liaison for VA's Health Care for Homeless Veterans Programs, Homeless Providers Grant and Per Diem Program. The Social Work Program Coordinator is assigned to Audie L. Murphy Memorial Veterans' Hospital and provides services to Veterans throughout the South Texas Veterans Health Care System catchment area.

The Grant and Per Diem Program, an essential and critical part of VHA, is vital for providing safe transitional housing and supportive services for homeless Veterans. VA medical centers must ensure that Veterans in community-based funded programs are receiving quality services. The Social Work Program Coordinator is responsible for monitoring the GPD grantee, providing oversight of these services and training as needed regarding appropriate supportive service practices to assist homeless Veterans. The incumbent provides services and grant oversight of the Homeless Grant and Per Diem funded programs as defined in the Grant and Per Diem rules, regulations, VHA Directive 1162.01 add local GPD Standard Operating Procedure (SOP) 122-HCHV GPD-22-01. They develop policies, procedures, and/or practice guidelines for program operation; and are able to organize work by setting program priorities, meeting deadlines, evaluating program needs, and preparing reports for facility, VISN, and national use. They may be responsible for the program's budget or contribute to, the program's resource and fiscal management by monitoring functional control points, developing the annual budget, operating within that budget, and accounting for appropriated funds. Social Work Program Coordinator authorizes per diem payments in compliance with the procedures outlined in Homeless Providers Grant and Per Diem Program directive and local SOPs. The Social Work Program Coordinator is assigned to administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. They oversee program operations and evaluations, identify areas for improvement, gather relevant data, assess the data, develop and implement ideas for improvement and evaluate efficacy of improvement efforts. The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseload and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. The incumbent participates in Medical Center research by identifying questions and trends that should be studied as a basis toward improving aspects of social service and/or physical or mental health care delivery to Veterans as well as devising and carrying out studies designed to test appropriate hypotheses. The incumbent monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application. When necessary, the incumbent intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates, and monitors plans of correction. The incumbent coordinates and participates in program site inspections utilizing the Grant and Per Diem Program rules, regulations, directives and local SOPs as guidance and forwards completed inspection packages to the appropriate offices. When necessary, the incumbent notes inspection deficiencies and establishes methods to monitor corrective actions and develops timelines for consequences as a result of inaction or insufficient actions related to deficiencies. The incumbent actively participates in the Grant and Per Diem Program conference calls and attends training offered by the Grant and Per Diem Program. Participating in committee assignments, work groups, and task forces at the facility and VISN level or in the community as deemed necessary by the service manager. The incumbent takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. The incumbent provides case management at Homeless Grant and Per Diem Program sites as described in the Homeless Grant and Per Diem program rules, regulations, directive and local SOPs. When a GPD Case Manager is not assigned to the GPD program the GPS Liaison is responsible for case management responsibilities. Functioning as lead coordinator for assigned Veterans who are enrolled in Care Coordination & Integrated Case Management. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12: In addition to the basic requirements above, the following criteria must be met when determining the grade level. Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice.
