This position is located within the Fort Yuma Service Unit. Incumbent is responsible to provide education to individuals and families on a wide variety of topics with an emphasis on diabetes education. Collects and assesses data related to patient health status to develop and evaluate the plan of care coordination needs across settings with the patient, family, and/or other care providers. The incumbent reports to the Chief Nurse Executive. Position is located in Winterhaven, CA.
Duties
Delivers comprehensive and seamless services to integrate clinical and self-management in the aspects of diabetes and care into clinical practice.
Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings based on assessed needs.
Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardio-metabolic conditions.
Track community diabetic health status, evaluate health trends, and risk factors of diabetic population groups. Collect data in a systemic and organized manner and analyze data. Collaborate with community leaders to evaluate the data, determine opportunities and needs for
the future.
Provide diabetes education and program services in orientation for new personnel related health workers, personnel on training programs and special projects personnel. Plan and implement training programs for trainees, co-steps and summer students.
Certified Diabetes Care and Education Specialist or BC-ADM credential preferred; if not already certified upon hire, employee will obtain certification within two years of appointment.

Qualifications
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.

BASIC REQUIREMENT(S):
Education:A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS - GS-11:
One (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Identifying patients' needs and assisting the patient and the patient's family in establishing treatment goals; Writing updates and revising care plans for patients in collaboration with other teams members; Participating in the diabetes program with multidisciplinary team to assess and provide culturally relevant care for patients living with diabetes; Providing technical assistance with guidance in planning, implementing and evaluation of continuing education training programs on a continuous basis; OR
PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
