This position is located in the Pharmacy Department at the Ft. Duchesne Health Center, in Ft. Duchesne, UT. The purpose of this position is to serve as an Advanced Practice Pharmacist (non-supervisory) providing direct pharmaceutical care in an outpatient setting in not only the screening, dispensing role, but in the advanced role of independently managing patient drug therapy under collaborative practice agreements (e.g. diabetes, smoking cessation, asthma, dialysis, etc.).

Duties

Takes an appropriate medication history, performs medication reconciliation, and performs limited physical assessment related to the specialized pharmacy clinics. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems. Evaluates medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug events. Administers immunizations to patients within the scope of practice set for by the state of Utah. Ensures accurate filling and dispensing of prescription and over-the-counter drugs on orders of physicians, optometrists, physician assistants, nurse practitioners, and dentists. Assures the availability (procures cost-effective, quality drugs for timely dispensing), preparation (prepackages and bulk compounds for departmental efficiency), and removal of outdated drugs from the facility. Understands and utilizes current technology regarding clinical pharmacy and pharmacy management tools available through personal computers, Resource Programs Management Systems (RPMS), and other automated pharmacy dispensing cabinetry and robotic systems. Provides formal and informal education and in-services to medical staff, pharmacy, and nursing staff as requested. Provides education to patients and their families about medications, common disease states and information on health promotion. Performs drug usage evaluations (DUE), quality assurance/performance improvement (QAPI) activities, and drug formulary review evaluations.

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENTS: Note: Employees currently assigned to positions in this occupational series (Pharmacy Series, 0660) as of September 2017 will be considered to have met the basic requirements for the position occupied. Education: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Licensure:All applicants must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE). Applicants also must be licensed to practice pharmacy in the State, the District of Columbia, and the Commonwealth of Puerto Rico, or a territory of the United States. Medical Requirements: Applicants must be able to distinguish colors. Experience: Your qualifications and background must demonstrate the knowledge, skills, abilities, and competencies necessary to perform the work of the position. Pharmacy work requires knowledge of the use, clinical effects, and composition of medications, including their chemical, biological, and physical properties. Qualifying professional pharmacy experience may involve, but is not limited to: Dispensing medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects; Evaluating medication use patterns and outcomes for patients in hospitals or managed care organizations; Performing administrative, consultative, or staff advisory work for a medical facility's pharmacy program; Planning, monitoring and evaluating medication programs or regimens; Establishing medication-handling procedures for the storage and preservation of medications; Researching medical literature and/or clinical medication information to provide accurate responses to inquiries; and/or Maintaining all medication records required by law. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS, GS-0660-11: Three (3) years of progressively higher level graduate education leading to a Ph.D. degree, Ph.D., Pharm.D. or equivalent doctoral degree. MINIMUM QUALIFICATIONS, GS-0660-12: In addition to meeting the requirements for the GS-11 (or equivalent) level, applicants must have a minimum of year of professional pharmacy experience. Examples include: Reviewing and optimizing pharmaceutical treatment plans by collaborating with medical staff making supportable clinical recommendations; serving as a clinical pharmacist to a medical home care team; providing continuing education presentations to medical staff; providing anticoagulation and tobacco cessation services; performing clinical pharmacist duties of chronic care management utilizing granted medical staff privileges; and providing population health care and preventative health. You must meet all qualification requirements within 30 days of the closing date of the announcement.
