The Home Based Primary Care (HBPC) Social Worker interprets and implements Social Work Service/Mental Health & Behavioral Science as well as HBPC/Geriatrics & Extended Care (GEC) policies and procedures and interprets and supports the overall policies and procedures of the West Palm Beach VA Medical Center.

Duties

The HBPC Program Social Worker, as an independent Social Work practitioner, is responsible for case management as well as psychosocial assessment and treatment services for all patients in the West Palm Beach VA Medical Center. The HBPC Social Worker will collaborate closely with the other members of the HBPC Program. Duties and responsibilities include, but are not limited to: Developing, maintaining, and evaluating social work services for HBPC patients. Initiating changes and modifications that will promote efficient practice. Providing direct practice and counseling for those patients with complex problems and coordinating psychosocial services for Veterans in the HBPC Program. Managing a workload including new, on-going and departing patients and establishing priorities for timely service and delivery of services. Recording social work activity in the electronic medical record promptly, accurately, and concisely. Participating in additional program planning addressing complex medical and social problems, identifying program and MH&BS rounds, staff meetings, in-service education programs and committees. Developing and maintaining screening methods for HBPC patients who could benefit from in-depth social work assessment and intervention. Utilizing standardized screening tools to assess cognition, depression, and caregiver stress. Examples include patients experiencing behavioral issues, those active with Adult Protective Services, Veterans needing placement in a community nursing facility, Veterans in need of or receiving community respite, and those who would benefit from end of life planning. Assessing the psychosocial and environmental needs of HBPC patients to generate solutions to identified problems. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-11 Social Workercandidates must have the following: Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Preferred Experience: Patient-Aligned Care Team (PACT), medical social work, geriatric care experience References: VA Regulations, specifically VA Handbook 5005/120, Part II, Appendix G39, Social Worker Qualification Standard. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive/operate government vehicle.
