Incumbent serves as supervisor and is administratively and clinically responsible for the following clinical programs; Veteran Directed Care and Community Home Health to include both skilled and non-skilled services.

Duties

Major duties include: Provides leadership, guidance, and direction for the clinical and administrative areas of the non-institutional care Geriatrics and Extended Care (GEC) programs to include, but not limited to, Community Home Health and Veteran Directed Care (VDC) Programs and staff. Develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services based on VHA Handbooks, Directives, IVC Field Guidebook, Memorandums, etc. Collaborates with community partners to streamline and maintain the most appropriate level of care to meet patient needs. Conducts training for staff, students and community providers as needed and will provide education and information about resources for Veterans in the community. Provides supervision for social work licensure and/or serve as clinical supervisor for social work graduate students. Ensures adequate program coverage and makes changes accordingly. Ensures that work is completed in a timely manner and reflects quality documentation in accordance with Service, Medical Center, VA and Joint Commission policy and standards. Provides back up coverage to other Social Work Service programs including but not limited to the Community Nursing Home Program, Adult Day Health Care Program, Caregiver Support, and Medical Foster Home programs. Provides consultation services to other staff regarding the biopsychosocial needs of Veterans and the impact of biopsychosocial problems on health care and adherence to treatment plan. Qualifications

Basic Requirements Citizenship: Citizen of the United States. Licensure: Candidate must be licensed or certified by a state at the advanced practice level, and must be able to provide supervision for licensure. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Preferred Experience: One or more years of experience working in geriatrics and extended care and familiarity with purchased care programs is preferred. Grade Determinations: GS-12 Supervisory Social Worker Experience: In addition to the basic requirements, applicants must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, candidates must demonstrate all of the following KSAs: Ability to independently organize work, set priorities, and meet multiple deadlines. Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. Ability to ensure provision of clinical social work services. Ability to provide staff training and development. Skill in interpersonal relationships in dealing with employees, team leaders, and managers Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. At the GS-12 grade level, supervisory social workers typically have significant personnel management responsibilities for a small to moderate sized professional and non-professional staff, including support staff and professional staff at the GS-12 grade level and below. The supervisory social worker is responsible for the professional and administrative management of an area in a social work service, in a care/product line, or multiple facilities. They provide direct clinical services with or without program coordination responsibilities. Must have a valid driver's license and be able to drive a government vehicle.
