The Social Work Coordinator is the Admissions/transfer Coordinator for The Domiciliary who has experience that demonstrates possession of advanced practice skills and judgement in clinical services for Veterans that suffer with mental health/dual diagnoses and substance abuse disorders.

Specifically, the Social Work Coordinator supports the VA Domiciliary Program by providing continuity of care for Veterans between VHA and community-based programs, as the Veteran transitions from Domiciliary care into programs such as HCHV Contract Residential Housing (CRS), Residential Rehabilitation and Treatment Programs (RRTPs), and Grant Per Diem Programs (GPD) as well as directly to permanent housing through the Housing and Urban Development Veterans Affairs Supportive Housing (HUD-VASH) Program, the Supportive Services for Veteran Families (SSVF), or other forms of housing in the community. The Social Work Coordinator provides leadership in the Domiciliary as well as the Community by actively participating in the local plans to address mental health treatment/coordination in Veterans. The Social Work Coordinator must be able to develop appropriate professional relationships with community stakeholders and is responsible for communicating policy or protocol changes to local VA leadership where such changes would enhance the VA's ability to coordinate housing and services for Veterans within the Domiciliary. The Social Worker will also function as a liaison, participating in case conferencing and other pertinent community meetings, as needed. Additional duties include but not limited to: Evaluates veterans' appropriateness for admission, manages a waitlist, acts as a liaison to referring clinicians, and coordinates the admissions process. Must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans as they transition from the Domiciliary to appropriate discharge alternatives. Care management responsibilities include the management, coordination, and provision of social work and supportive services to Veterans and/or their families, including provision of individual and group counseling to Veterans for issues related to Substance Abuse Disorders and other complex and diverse psychosocial problems. Is a member of an interdisciplinary team and is responsible for active participation in interdisciplinary treatment team meetings and resident centered care planning. Independently work with Veterans who are experiencing issues related to Mental Health/Substance Abuse Disorders who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. Actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of a resident centered treatment plan. Possess a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology. Facilitate discharge planning through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate discharge plans are executed in a timely manner. Serve as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, with rotating tours of duty as needed. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker (Program Coordinator) candidates must have the following: Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
