The Pharmacist HBPC will support a newly formed second team in the Fayetteville catchment area. The Pharmacist will work directly with the provider and the other HBPC Interdisciplinary team members to provide care to geriatric patients in the Fayetteville and surrounding areas within a 50 mile radius. The Clinical Pharmacy Specialist will independently manage unstable pharmacotherapy patients in his/her pharmacotherapy clinics, improving the clinic system efficiency and quality.

Duties

The Clinical Pharmacy Specialist (CPS) Home Based Primary Care (HBPC) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS HBPC plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Major Duties to include but not limited to the following: Coordinates all clinical pharmacy functions for Fayetteville HCC Home Based Primary Care (HBPC) program to include those related to the Rural Health Initiative and works with other team members in solving all drug-related problems. Demonstrates expertise in the area of geriatrics and the goals and objectives of the HBPC program. Attends and participates in HBPC multidisciplinary rounds as needed, attends team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters. Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations as necessary. Identifies patient-specific medication issues, including drug interactions, adverse effects, lack of effectiveness, appropriateness, and compliance problems. Supervises pharmacy trainees assigned to the program. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required ensuring appropriate and safe drug utilization. Actively participates in local, state and national organizations related to the advancement of the pharmaceutical profession. Clinically reports directly to and requires minimal supervision by Clinical Coordinator Pharmacy services. Administratively reports to HCC Supervisor. Education Provides formal education and training programs in the area of cardiac pharmacology, therapeutics, pharmacokinetics and related subjects to physicians, nursing staff and other health care professionals as appropriate. Contributes to Pharmacy Newsletters and other written educational written educational materials as assigned. Participates in P&T Committee functions and assignments as required ,e.g., ADR monitoring, medication error reporting, Medication Utilization Evaluation activities, drug monographs/reviews, etc. Attends and actively participates in all clinical pharmacy activities, i.e., staff development, residency conferences, journal club. Provides drug information and drug literature evaluations as requested by the professional staff. Prepares and presents educational lectures to a variety of disciplines as required. Provides formal education and clinical education in the area of expertise to pharmacists in training. Serves as a Pharmacotherapy Manager mentor to medical students, residents, and physician extenders, teaching them pharmacotherapy and healthcare economics. Program Management Responsible for coordination and supervision of all pharmacotherapy management activities as assigned. This includes management of a clinical practice within area of expertise including scheduling, reviewing consults, finishing encounters to meet timeliness standards, providing patient care, and assuring coverage of clinical services. If scheduling authority is requested and granted the incumbent schedules outpatient clinic appointments using Veterans Health Information Systems and Technology Architecture (VistA) scheduling options. Uses established business rules to schedule appointments for veterans. This includes clinic visits, consultations and entries into the Electronic Wait List (EWL). Complies with the mandatory training requirements for the outpatient scheduling process. Quality Management Participates in the appropriate, effective and safe use of pharmaceuticals by contributing to criteria development, data collection and drug use evaluation and other quality improvement activities as assigned. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education A Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate can provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: (1) Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. (2) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. (3) If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations Pharmacist (HBPC Clinical Specialist ), GS-13: In addition to the basic requirements for employment stated above, the following criteria must be met when determining the grade of candidates. Experience: One year of creditable experience equivalent to the next lower grade level (GS-12). Qualifying GS-12 Clinical Pharmacist grade level experience includes, but is not limited to: handling routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. AND; Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: PGY1 Pharmacy Practice Residency PGY2 Pharmacy Practice Residency BCPS Certified (Ambulatory Care, Pharmacotherapy or Geriatrics) Experience As A Pharmacist In A Clinical Setting References: VA Regulations, Specifically VA Handbook 5005/55, Part II, Appendix G15 Pharmacist Qualification Standard.
