The position is located within the Primary Care Directorate of the Captain James A. Lovell Federal Health Care Center in North Chicago, Illinois. This is the nation's first fully integrated federal health care facility, Integrating VA with the DOD, specifically, the United States Navy under a single chain of command. The incumbent performs a variety of duties in a complex 1C medical facility, which is constantly evolving within a highly competitive geographic area.

Duties

The incumbent serves as the PACT Social Worker Supervisor and assumes all of the technical and administrative responsibility for management of the social work services provided to PACT patient, This include but is not limited to : Provides administrative oversight of the PACT social work staff to include planning, establishing, and implementing policies, and procedures; developing program goals and objectives; and monitoring, operating, and evaluating the functioning of the program. Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for patients in PACT. Identifies unmet needs of the PACT patients to work collaboratively with. Explores and analyzes community resources including family- friendly delivery system. Ensures that patients are screened for the need for case management services and that those in need of nurse and/ social worker case managers are assigned accordingly particularly those who are severely injured or ill. Coordinates the assembly of interdisciplinary meetings and collaboration when needed. Performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Provides consultation to other staff on the special needs of PACT patient populations. Establishes and maintains an ongoing education program for patients, community agencies, student, and staff to facilitate understanding of medical treatment, including clinical practice guidelines, mental health issues, readjustment issues, psychosocial problems, and care giver issues. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience and the following KSAs . Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills. Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-39 Social Worker Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Eligible applicants must be physically and mentally able to perform efficiently the essential functions of the position, with or without reasonable accommodation, without hazard to themselves or others. Depending on the essential duties of a specific position, usable vision, color vision, hearing or speech may be required. However, in most cases, a specific physical condition or impairment of a specific function may be compensated for by the satisfactory use of a prosthesis or mechanical aid.

Education

Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
