The primary function of the Compounded Sterile Preparations (CSP) Pharmacist is to manage, develop, organize, and coordinate all aspects of compounded sterile preparations to include policy development, quality, resources, and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. The incumbent will coordinate a single program area of a complex pharmacy operation.

Duties may include but not limited to:
Serves as the designated staff to complete, and or facilitate compliance, the requirements set forth in VHA Directive 1108.12.
Responsible for oversight of USP compliance in the Pharmacy Service, Joint Commission Standards, FDA requirements and VHA polices related to CSP's.
Act to ensure that appropriate policies and procedures are written and in place to support full compliance with all standards and guidelines.
Develop a written contingency plan for use to obtain CSP's from an alternative source (outsource) in situations where CSP preparations at the facility need to be suspended, ensuring that the decision to outsource or to prepare CSP prescriptions or to prepare CSPs in the medical facility pharmacy follows the VHA PBM decision-making hierarchy and associated guidance.
Act to ensure the competency of Pharmacy staff for the preparation and handling of CSPs as required, and ensure that hazardous drugs are transported, stored, and handled by other health care professionals are appropriately labeled to alert staff to handle the HD's as required.
Maintain environmental control of the sterile compounding areas including collaborating with other services to ensure that primary and secondary engineering controls are certified, develop a formal written CSP safety and quality assurance plan in collaboration with facility QM & Infection Control and act to ensure that the facility's hazardous drug list is reviewed and updated annually.
Develop written procedures and organize and manage the Intravenous Admixture and Sterile Compounding program for full compliance with all applicable laws and regulations (e.g., Joint Commission, USP 797/800,VA/VISN, OIG, etc.).
Administering the competency of Pharmacy personnel in the preparation and handling of CSPs and maintaining environmental control of the sterile compounding areas through active and effective collaboration with Biomedical/Engineering/Facilities Management Services and Environmental Management Services, Quality Management, and the Infection Control to develop a formal written CSP Safety and QA plan in accordance with USP Standards.
A working knowledge of contemporary drug distribution systems including unit dose and I.V. admixture is required.
Demonstrate the skill to plan and oversee the implementation and monitoring of these pharmacy distribution systems and the staff necessary to perform the duties effectively and efficiently on a day-to-day basis.
A working knowledge of Excel, Word, PowerPoint is needed along with the capability to set up group meetings utilizing acceptable facility venues.
A required participant on the facility USP 797/800 work group and may be assigned to serve as the secretary of the committee.
Serves on the facility's committee(s) as assigned and provides updates related to IV room quality assurance data to the Pharmacy and Therapeutic Committee, Quality Leadership Committee and or Clinical Executive Committee or other regulatory/oversight bodies as necessary.
Demonstrate knowledge of professional pharmacy practice, ability to communicate orally and in writing to persuade and influence clinical and management decisions, expert understanding of regulatory and quality standards for this program, ability to solve problems, coordinate, and organize responsibilities to maximize outcomes in their program area or area of clinical expertise, expert knowledge of a specialized area of pharmacy, and advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Meet the Pharmacist qualification standards per VA Handbook 5005/55 and demonstrate thorough knowledge and understanding of various governmental regulations, including FDA, DEA, USP, and Joint Commission, pertaining to the practice of pharmacy is required along with standard operating procedure manuals, and VA National office directives.
In accordance, the incumbent must be a licensed pharmacist with extensive professional experience and broad knowledge of all aspects of the related Pharmacy Service operation.
Must maintain full and active pharmacist licensure to practice pharmacy and attain the required ACPE continuing education credits to do so.
Must also maintain American Heart Association Basic Life Support (BLS) certification as a condition for continued employment at the John D. Dingell Medical Center.
Work Schedule: Monday - Friday, 8am-4:30pm, with rotating tours as well as weekends and holidays to assess workload

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Grandfathering Provision: All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: (1) Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
(2) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
(3) If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Grade Determinations: (GS-13)
Experience: In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level(GS-12).
Must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Board Certification by the National Board of Pharmaceutical Specialties
Minimum 1 year experience in management and monitoring of pharmaceutical compounded preparations

The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is GS-13.
Physical Requirements: Must be able to distinguish basic colors with ability to distinguish shades of color. This position may require heavy lifting and carrying, 45 pounds and over. Requires routine standing, bending, and stooping/squatting. As needed - pushing, walking, standing, reaching over shoulder, use of fingers, both hands and legs required, walking and standing up to four hours per tour, may include use of stairs. This position will require the candidate to read computer screens and other documents for extended period of times, up to 4 to5 hours consecutively near vision correctable at 13' to 16' to Jaeer 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, hearing (aid permitted). At times, candidate may be around machinery with moving parts. The job entails working closely with others and at times alone. The work environment is inside a pharmacy -there may be extremes of temperature, distractions, noise from equipment or patient care activities. May need to handle frozen and refrigerated products. Exposure to negative and positive air pressure rooms is possible. Twisting and grasping of bottles and vials or syringes can occur. Must be able to don and work with protective garments and equipment as necessary.
