Pain Management APRN provides specialty care pain services to a panel of patients through a combination of virtual and in-person visits as part of an interprofessional specialty care pain team in the "Biopsychosocial Model of Pain Care" and "VA Stepped Care Model of Pain Care" model. The incumbent works as part of a virtual and in-person specialty pain care team, together with patient site care team members, to provide specialty pain care to Veterans.
Duties
Major Duties:
Perform initial history and physical examinations. Completing history and physical examinations via direct face-to-face or TeleHealth visits.
Provide ongoing chronic pain care for assigned patients.
Screen patients to determine need for further care.
Prescribe medications within defined scope of practice.
Utilize evidence-based approach to medication management for chronic and acute pain.
Provide evidence-based approach to medication management for Veterans with Opioid Use Disorder (OUD).
Order and interpretation of diagnostic studies such as laboratory tests, x-rays, magnetic resonance imaging (MRIs).
Review opioid safety metrics (e.g., PDMP, drug screening, STORM reviews).
Initiate, respond, and expedite request for consultations, special tests, and studies.
Provide education and counseling for patients and families in chronic pain care, medical conditions, and use of prescribed treatments and drugs.
As appropriate, participate in interprofessional focused patient chart reviews, team meetings, team huddles, and shared medical appointments.
Clinical Responsibilities:
May occasionally perform interventional pain medicine procedures in designated VA facilities as part of a balanced, data-driven, multi-disciplinary treatment plan, in accordance to current licensing and standards of care.
Provide electronic consultation services.
Perform pain management comprehensive assessments and develops treatment plans for complex acute and chronic pain patients.
Manage opioid medications and perform opioid tapers and detoxification when necessary. Comply with current VA/DOD Long-term Opioid Therapy Guidelines, and local policies.
Treat patients with chronic and acute pain.
Develop therapeutic care plans that are intended to achieve defined outcomes that improve the patient's quality of life.
Work as part of an interdisciplinary team of pain management providers.
Initiate, modify, or discontinue medication therapies and serve as a consultant for intensive medication therapy management services.
Order, perform, review, and analyze appropriate laboratory tests, other diagnostic studies and registries necessary to monitor and support medication therapy.
Follow up with patients regarding laboratory or other diagnostic test results to discuss care plans, changes to therapy and additional monitoring or testing requirements.
Evaluate and reports adverse outcomes via local VA standard reporting processes.
Document all pain management activities, including therapeutic changes and recommendations, in progress notes and completes encounters per VHA standards to ensure workload capture.

Safeguards patient privacy and maintains confidentiality of all electronic and printed patient information according to HIPAA guidelines, the Privacy Act, and as stated in the Medical Center Policy. Maintains confidentiality of assigned access code and by exiting (logging out) from the computer system when leaving terminal. Does not share security codes and does not allow others to use the computer under own security codes. Maintains patient confidentiality by not discussing patient information except when necessary to authorized staff only in order to meet health care needs. Ensures application of institutional policy to protect information from unauthorized release, loss, alteration, or deletion. Does not use thumb-drives or other devices.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.

Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
In addition, this position requires a pre-employment physical. Requires lifting 15-44 pounds; pushing (approx. 2 hours); reaching above shoulder; use of fingers and both hands; walking and standing from 3-5 hours and kneeling. Ability for rapid mental and muscular coordination simultaneously. Near vision correctable at 13" to 16" to Jaeger 1 to 4. Far visions correctable in one eye to 20/20 and to 20/40 in the other. Must have depth perception and ability to distinguish basic colors and shades of colors. Hearing aid is permitted.
