This position is located within Nursing Services, Four Corners Regional Health Center, in Red Mesa, AZ. Incumbent will provide diabetes education services, Nursing expertise and leadership in assessing, implementing, and evaluating care provided to individuals with or at risk for Type 1 & Type 2 diabetes and diabetes in pregnancy. Provides management education, and prevention to those at risk for diabetes of all age groups. Provide community-based diabetes intervention and case management.
Duties
Provides outpatient education and counseling to individuals and families according to curriculum, policies and procedures of the facility and the Diabetes Program.
Serves as a primary instructor for individual and group classes.
Coordinates diabetes care and patient visits with the appropriate departments and clinical staff including; Family Medicine, Pediatrics, Urgent Care, Dental, Physical Therapy and Optometry, for prevention of diabetes care and education.
Provides individual and group education as necessary and/or as requested by clinical staff on diabetes care (provider oriented), self-care and management (client oriented). Works closely with the Diabetes Team to update and maintain an accurate patient registry; follow up patients who have not been seen within the past 12 months.
Assists the department director with development and revision of policies an procedures relating to diabetes and participates in performance improvement activities.
Designs, develops, coordinates, implements, manages, and evaluates special projects independently to improve the diabetes program performance and submits reports on a timely basis on performance improvement, training, continued education.
Qualifications

BASIC REQUIREMENT(S):
Education:
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure:
For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
MINIMUM QUALIFICATIONS:
Experience: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Examples include: Utilizes existing and self-developed teaching tools and course material and class population and consults with other members of the educational department team for their input into specialized training programs. Assesses learning and training needs of newly licensed nursing staff and contract/agency staff utilizing a variety of tools from which recommendations for in-service education, specialized training, individualized training, and new and improve training programs can be made. Collects and organized data obtained from assessment methods. Teaches and/or directs and coordinates others to teach utilizing new or adapted approaches. Utilizes appropriate teaching methodology and leveling techniques to ensure course effectiveness.
GS-11:
In addition to the mandatory license and education requirements, applicants must meet one of the following requirements:
Successful completion of a PHD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
