VA San Diego is hiring a Senior Social Worker to work as a Case Manager in VA Supportive Housing (VASH) under the Health Care for Homeless Veterans Program. VASH is a collaborative effort supported through the Department of Housing and Urban Development Section 8 rental assistance program and VA's provision of intensive case management services. The program is based at a Community Based Outpatient Clinic, but services are provided to clients primarily in the community.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU You will serve as a Senior Social Worker (Case Manager) for VA Supportive Housing (VASH) under the Health Care for Homeless Veterans (HCHV) program. The primary component of the program is VA case management services designed to improve Veterans' health and mental health, and enhance their ability to remain stable, housed and community integrated. Your primary responsibilities will include clinical and administrative program oversight and monitoring; serving as one of the primary contacts with the local Public Housing Authorities (PHAs); providing clinical and psychosocial case management services; and ensuring that the Veterans' care is coordinated with the VA Medical Center, community health or mental health providers, and agencies that serve homeless populations. Duties include, but are not limited to: Functioning as case manager for Veterans in the HCHV/VASH program, performing duties to facilitate transition to independent community living by coordinating mental health and community care. Implementing and maintaining referral and screening procedures for potential VASH Veteran participants. Acting as a liaison to the PHAs, community landlords and other community agencies that serve homeless populations as well as developing working relationships and agreements with other organizations and directly coordinates with the designated PHAs regarding housing voucher applications and the award process for each Veteran. Evaluation the Veteran's situation, medical and psychiatric stability, and abilities and capabilities and arrives at a reasoned conclusion for admission. Developing psychosocial treatment plans. Providing case management services including ongoing assessment, education, coaching, counseling, referral and advocacy. Coordinating care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linkage and referral of Veterans to the VA Medical Center, VA Regional Office and/or other community agencies. Providing direct mental health and substance abuse services for Veterans and family members or significant others in support of the Veteran's treatment. Participating in program planning, development and evaluation. Providing orientation and on-going training to social workers and interdisciplinary team members. Serving as a mentor to other social workers, particularly those at the entry level, and may provide clinical supervision for unlicensed social workers and social work associates. Coordinating and participating in special homeless events. Work Schedule: Monday - Friday, 7:00am-3:30pm Compressed/Flexible:Compressed schedule may be available. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Must possess a valid driver's license and must qualify to transport patients. Grade Determinations: In addition to meeting the basic requirements above, applicants must meet the below grade requirements for a GS-12 Senior Social Worker. Experience/Education: Must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification: Must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate the following KSAs (NOTE: experience must be listed in your resume in order to receive credit): Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
