The incumbent practices as an Nurse Practitioner on the Referral Coordination Team (RCT) and assists with consult processing for various categories of care. Will work with the RCT to review the medical necessity of services requested, available VA and community resources, determine community care eligibility, and work with the multidisciplinary team to coordinate specialty care referrals. Accountable for the evaluation, management, and coordination of patient-driven care.
Duties
Reviews consult requests.
Identify available referral routes, facilitating recommendations from specialty providers (e-consults).
Order and interpret diagnostic and therapeutic tests/studies, prescribe medications, and supplies in accordance with VA policy, VA formulary, and State regulations, and formulate and evaluate healthcare teaching for patients and their families.
Guide, develop, and support staff from a leadership perspective.
Use professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities. The employee will be a clinical surrogate for licensed independent provider(s) within their assigned work group.
A patient-centered approach with emphasis on preventative strategies and patient education, plus flexibility in facilitating care through various modalities, are required.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Preferred Experience: Knowledge of the referral and scheduling processes of both internal/direct VA and community care. Experience reviewing and triaging referrals and discussing all available options for care locally, virtually, in other VA locations, and community care based on eligibility.
Grade Determination.
In addition to the basic qualification requirements, the grade level of Nurse (any assignment) is based on a combination of education and experience. Beyond the minimum education requirements, grade
determination for appointment, advancement and promotion will be based on the following specific requirement for Nurses:
(1) Nurse I, Level 1.
Education and Experience Requirement. Diploma or associate degree in nursing with no professional nursing experience.
(2) Nurse I, Level 2.
Education and Experience Requirement.
i Diploma or associate degree in nursing and one year of specialized nursing experience equivalent to a Nurse I, Level 1
OR
ii Bachelor's degree in nursing and no professional nursing experience.
(3) Nurse I, Level 3.
Education and Experience Requirement.
i Diploma or associate degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 2.
OR
ii Bachelor's degree in nursing and one year of professional nursing experience equivalent to Nurse I, Level 2.
OR
iii Master's degree in nursing and no professional nursing experience.
OR
iv Master's degree in a related field with a bachelor's degree in nursing with no professional nursing experience.
(4) Nurse II.
Education and Experience Requirement.
i Bachelor's degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 3.

ii Master's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3.
OR
iii Master's degree in a related field with a bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3.

iv Doctorate degree in nursing with no professional nursing experience.
OR
v Doctorate degree in a related field with a master's or bachelor's degree in nursing with no professional nursing experience.
(5) Nurse III.
Education and Experience Requirement.
i Master's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III.

ii Master's degree in a related field with a bachelor's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension
requirements for Nurse III.

iii Doctorate degree in nursing with one year of specialized nursing experience which is equivalent to Nurse II and meets all dimension requirements for Nurse III.

iv Doctorate degree in a related field with a master's or bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for
Nurse III.

Physical Requirements: The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. James H. Quillen Veterans Affairs Medical Center (JHQVAMC) is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, lifting 10 pounds or more, and pushing. Transferring patients and objects may be required. The RN may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The RN may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The RN must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
