Duties
This position is located in the Primary Care Division, Whole Health at the Clement J. Zablocki VA Medical Center. This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing, and Whole Health clinical care elements of the Whole Health System, as well as assisting in the collection of whole health qualitative data from Veterans and creation of reports analyzing effectiveness of health coaching.
Duties:
-Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones.
- Contacts eligible participants for enrollment and explains the health coaching process
- Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings.
- Conducts Whole Health Facilitated Groups; MOVE, Smoking Cessation, Diabetes Management, Hypertension Clinics, Mental Health, and other inpatient and outpatient groups as appropriate.
- Provides follow-up care through telephone clinics, secure messaging, or face-to-face appointments- Actively guides Veterans in the exploration of core elements of personal health planning through individual or group based programs
- Reviews current health behaviors and develops customized treatment plans with Veterans
- Utilizes motivational interviewing and health coaching techniques and processes

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 02/26/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-5, and for a GS-9 you must have severed 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience and/or education as described below:
Basic Requirements:
Degree in behavioral or social science; or related disciplines appropriate to the position.
OR
Combination of education and experience -- that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
OR
Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
NOTE: In addition to meeting the above qualification requirements, applicants must have specialized experience and/or directly related education in the following amounts.
GS-7:Specialized Experience:
You must possess at least one year of specialized experience at the GS 5 level, that has equipped you with the knowledge, skills and abilities to perform successfully the duties that is typically in or related to the work of the position to be filled. Examples of specialized experience include but are not limited to demonstrated experience in: Working with health coaching techniques, motivational interviewing and health coaching techniques and process; obtaining specific factual data or opinions on specific issues; providing information regarding weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, and pain management.
OR
Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed 1 full year of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree related to this position.
OR,
Superior Academic Achievement. Education must be in behavioral or social science; or related disciplines appropriate to the position. (Transcripts Required)

Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond completion of undergraduate level work. Education must be related to the position. (Transcripts Required)
GS-9:Specialized Experience:
You must possess at least one year of specialized experience at the GS 7 level, that has equipped you with the knowledge, skills and abilities to perform successfully the duties that is typically in or related to the work of the position to be filled. Examples of specialized experience include but are not limited to: Working with health coaching techniques, motivational interviewing and health coaching techniques and process; obtaining specific factual data or opinions on specific issues; providing information regarding weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, and pain management. Developing and conducting onsite educational series for enrolled patients. Coordinating care with treatment providers. Retrieving, tracking and trending information needed for reporting related to health coaching and working with Quality improvement analysis techniques and methods.

Must possess two (2) years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree. Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another. Some physical effort such as standing, walking, bending or prolonged sitting may be required. The work places no special physical demands on the employee.
