Clinic Orders Pharmacist interprets, evaluates, and verifies provider medication orders. Other functions include assisting with the expansion of the Clinic Orders Pharmacy Program throughout the health care system and serving as a liaison between Pharmacy Service and other departments with regards to the Clinic Orders program.

Duties include but are not limited to the following:
Clinical Duties:
Completes medication histories.
Documents activities in CPRS.
Reconciles medication including updating the medication profile to reflect an accurate, active list of VA and non-VA medications.
Holds medication when necessary.
Generates unsigned discontinued orders.
Monitors drug therapy.
Assesses drug safety and efficacy.
Reports adverse drug events, near misses and medication errors.
Recommends formulary alternatives.
Provides oral and written consultations with other healthcare professionals regarding drug therapy selection and management.
Participates in medical emergencies.
Provides pharmacokinetic consultation including but not limited to renal or hepatic dosing adjustments in accordance with local policy.
Assists with drug recalls and medication shortage situations.
Participates in research in accordance with FDA guidelines and regulations and VHA policy.
Supportive Duties:
Applies knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics to monitor and assess patient's therapies, progress, labs, and drug-related problems.
Consults with other health care professionals to ensure the identification, resolution, and prevention of potential and actual drug-related problems.
Directs patient care activities, including monitoring of drug therapy and adjustment of medication regimen or dose under protocol for outpatient, discharge, and hospitalized patients.
Documents direct patient care activities using the electronic health medical record.
Distributive Duties:
Screens medication orders and dispenses medications.
Supervises, coordinates, and checks work performed by pharmacy technicians, students, and volunteers.
Utilizes electronic health medical record (EHR) for drug order and prescription entry, as well as retrieval of laboratory results and other patient-related data.
Coordinates the patient discharge process with regards to medication, to ensure that the necessary medications are supplied.
Ensures that professional standards, regulations, laws, and applicable guidelines and protocols are met when dispensing medications.
Operates and maintains the automated dispensing packaging and labeling equipment.
Education:
Participates in the training and orientation of pharmacy students, pharmacy residents, pharmacy staff and allied healthcare professionals.
Contributes information for written publication in newsletters and bulletins.
Participate in VA Pharmacy staff development presentations or in-services.
Administrative Duties:
Assists in gathering data and preparing studies of cost, efficiency, quality improvement, workload, and drug evaluations.
Keeps supervisor apprised of changes in workload, manpower requirements, resource availability, patient requirements, and developments.
Supports Pharmacy strategic planning decisions.
Assists staff members daily in the performance of regular pharmacist duties.
Assists all VANCHCS departments in collection of data as needed for management reports.
Contributes to the assessment, planning, and evaluation of the Clinic Orders Pharmacy Program with Pharmacy administration to ensure coordination between the delivery of pharmaceutical services and the overall delivery of health care.
Ensures that the Clinic Orders Pharmacy Program meets VA, Joint Commission, DEA and professional pharmacy standards/regulations/laws.
Attends and participates in regular clinic, Pharmacy Service, and committee meetings.
Works in cooperation with facility providers in the resolution of patient-related problems involving Pharmacy Clinic Orders.
Work Schedule: Monday thru Friday, Variable daytime hours.

Basic Requirements
U.S. Citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Education.
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy).
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
Grade Determinations: In addition to the basic requirements for employment stated above, the following criteria must be met.
Creditable Experience.
Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.
Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination)
Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.
Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.
Clinical Pharmacist, GS-12
Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
1 year of experience equivalent to the next lower grade level, or
Completion of an ACPE-accredited Pharm.D. program.
Knowledge, Skills, and Abilities( KSAs). Pharmacists assigned to this position must demonstrate the following KSAs:
Knowledge of professional pharmacy practice.
Ability to communicate orally and in writing to both patients and health care staff.
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.

The full performance level of this position is GS-12.
Physical Requirements: This position requires visual acuity (depth perception and ability to distinguish basic colors and shades of colors), near vision correctable at 13 to 16, and far vision correctable in one eye to 20/20 and 20/40 in the other and keen hearing. This position requires potentially long periods standing (6 hours) and you may be required to do light lifting and carrying of up to 15 pounds; reaching above shoulders. Both hands and legs are required along with the use of all fingers; hearing (aid permitted); repetitive computer use.
