The Dayton, Ohio Veterans Affairs Medical Center is recruiting for a well-qualified Pharmacist (Clinical Specialist- Women's Health PACT Pharmacist), within the Pharmacy Service. The Pharmacist supports Clinical Services as assigned, utilizing education, experience and expertise.
Duties
Duties include, but are not limited to:
designing, implementing and monitoring of therapeutic drug plans
obtaining medication histories from patients and summarizes significant findings
reviewing patient medication regimens
making dose adjustment recommendations
Reviewing and evaluating requests for non-formulary and restricted drugs
assisting with medication use evaluations
documenting clinical interventions in CPRS
promoting and monitoring compliance for established drug therapy policies; works with providers to ensure compliance
participating in the drug use evaluation process as needed
reviewing and verifying medication orders in assigned areas
primary provider to document information into Adverse Drug Event Reporting System
extending medication therapy until next medical appointment
consulting for intensive medication therapy management services
referring to the physician is necessary
training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other medical center personnel as required
instructing patients and family members in the appropriate use of medications and medical devices
assessing and documenting patient comprehension and to refer patients to educational programs provided
using both written and verbal communication techniques in teaching others
serving as preceptor for pharmacy students and pharmacy residents
assisting in new employee orientation sessions and in the development of proposals for improved and/or new clinical pharmacy services
serving as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians and students
maintaining a current knowledge of therapeutics and disease management
guiding other members of the staff in the implementation of the Chief of Pharmacy's intentions and/or policies
acting as an extension of the supervisor by assuming responsibility, seeking cooperation of employees and resolving problems within their ability
Work Schedule: 80 hours per pay period. Monday through Thursday, 0700 to 1630; alternate Fridays 0700 to 1530.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
Citizenship. Citizen of the United States.
Education
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
Grade Determinations:
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Ambulatory care and/or PGY-1 experience

Physical Requirements: Some of the work involves extended periods of time standing, sitting or doing computer work. This position requires average agility and dexterity. Professional appearances per pharmacy dress code, attitude and demeanor are required.
